A piece of flow line (1.5 tubing) is laying on the surface of the location. The site does not meet the final restoration requirements. The tank and separator are still at the entrance; however, Scott Davis stated that the landowner will sign a waiver to retain the tank and equipment.

0347065823 2016-04-15 Administrative Inspection No Violations At 10:07 am I called and spoke to Scott Davis, son of owner Dean Davis and field superintendent. Mr. Davis stated that the company has retained the services of Josh Thompson to facilitate the plugging of the well, by the end of the month. A permit to plug the well was issued in November of 2015. Reference Chiefs Order 2016-110.

The remaining equipment is to be left for the landowner based on a recent conversation with Scott Davis. I told Mr. Davis that a waiver form would be required to leave any equipment behind.

0701372554 2016-03-17 Production Wells No Violations My inspection today finds the well is still incapable of production. There are no pipelines attached to the well. The well is equipped with only 8.625 casing reduced to a two inch valve that is closed. 1.5 tubing and 5/8 rods are laying on location. The lease road has an erosion channel over 18 deep and forty feet long. The tank battery is equipped with one 100 barrel tank, one buried steel tank, and two horizontal separators. The 100 barrel tank is beginning to leak at the base. The tank appears to have approximately four feet of liquid in it based on feeling for temperature variances on the side of the tank. I spoke to Scott Davis with Green Gas. Mr. Davis is contesting ownership of the tank battery stating that the domestic owner of a nearby well that shared the production equipment is the owner of the tank. Further research is needed to determine the owner of the tank.
0740708187 2012-12-26 Production Wells Violations Noted My inspection today finds the well is equipped with 7 casing. The well head is not connected to any pipe line. All valves on the well are closed. There is a string of 1.5 tubing and rods on location. The tank battery consists of one 100 bbl tank, one 50 bbl tank, and two production units. The tank is shared with another well. The tank dike is not adequate. The well identification is not legible. No production has been reported since 1997. The well is incapable of production in its current state. WELL SHALL BE PLUGGED DUE TO NON PRODUCTION.

1277830408 2015-12-30 Production Wells No Violations My inspection today finds the status of the well has not changed. Only surface casing is visible. The valve at the top of the surface casing is closed and no pipelines are attached to the well. 1.5 tubing and rods are laying on location. The tank battery consists of one 100 barrel tank, one fifty barrel tank that is buried, and two separators. The 100 barrel tank is seeping at the base and small amount of oil is beginning to accumulate. I have requested enforcement action.
1380368849 2016-04-25 Plug / Plug Back No Violations On location to witness plugging operations for this well. This well has been plugged back to the Gordon Sandstone and total depth was tagged at 2450. To ensure a solid bottom plug was in place, a 100 plug was spotted in 7.875 open hole from 2350-2250 using 27 sks 1% gel cement and 8.7 bbl of freshwater displacement. Tubing was then pulled to 1700 and a 200 Berea Sandstone plug was spotted using 54 sks 2% gel cement and 5.7 bbl of freshwater displacement. Circulation was established at the end of plug displacement. Tubing was then pulled to 1256 and a 200 bottom-of-surface casing plug was spotted using 60 sks 1% gel cement and 4.1 bbl of freshwater displacement. Tubing was then pulled to 207 and the surface plug was spotted. Good cement returns were observed at the surface and the cement did not drop. All plugs were set through 2.375 tubing. LCM and CaCL were used on all plugs except for the surface plug.

1417933512 2015-09-29 Administrative Inspection No Violations At 2:29 pm I received a call from Scott Davis in response to a voice mail I left. Mr. Davis and I discussed the companys plans for the well. Mr. Davis stated that he was going plug the well. I reminded Mr. Davis that the well has been in violation for nearly three years and that the company may face escalated enforcement action if the violations are not addressed quickly.
1426728189 2016-04-25 Plug / Plug Back No Violations My inspection today finds the well has been plugged and the cement is visible four feet from surface. Thompson Well Service is removing their service equipment. I instructed Scott Davis to cut the casing off thirty inches below grade and reclaim the site in accordance with 1509.072(B). The previous violation has been resolved.
