0212935918 2014-07-22 Production Wells No Violations My inspection today finds Corps Well Service is removing the contaminated soil. I tested a spring 150 down hill of the well site for chlorides. The low range Quantab indicated the chlorides are at a level of 115 ppm. Once all visibly contaminated soil was removed I tested the soil with a low range Quantab. The test indicated chlorides in the soil were 174 ppm.

0382540634 2014-07-21 Production Wells Violations Noted At 1:00 PM I received a phone call from Mr. Joe Dimmerling the land surface owner. Mr. Dimmerling stated that there is oil and brine leaking from the well. I responded on site at 1:50 PM. Upon arrival I witnessed the pumping unit running and brine was leaking from the polish rod liner packing. The affected area was approximately 25 at the widest point and extended 75 from the wellhead. The estimated size of the release is five to ten gallons of crude oil and two to three barrels of brine I called Bill Arnholt with Northwood Energy and notified him of the leak at 1:56 PM. Mr. Arnholt stated that he would have Corps Well Service begin the clean up today. Corps Well Service brought six men and a dozer to begin the clean up. The company was instructed to remove and replace all contaminated soil and re-establish vegetation within fifteen days.

0693127786 2014-12-10 Production Wells Violations Noted At 8:38 AM I received a telephone call from Joe Dimmerling, the surface owner. Mr. Dimmerling stated that the well was leaking at the stuffing box. I arrived at the well site at 9:00 AM. Upon my arrival I witnessed approximately one to two gallons of very thick oil covering the wellhead and the ground covering an area two feet in diameter. I called Bill Arnholt, Vice President of Operations, with Northwood Energy. I informed Mr. Arnholt that the well was leaking and it required immediate action. I also informed Mr. Arnholt that the company was in violation of 1501:9-1-07(A). Suspend operation of the well until the leak is fixed and clean all oil from the well site.
0814885216 2014-06-02 Production Wells Violations Noted My inspection today finds the well is equipped to be produced by a pumping unit. A ten foot area around the well has been scalded by brine. A high range Quantab found the chloride levels eighteen inches from the well in excess of 6500 ppm. The tank battery is comprised of one 100 bbl tank, one 210 bbl tank, a separator and a meter set in an earthen dike. Remove all contaminated soil and replace it within fifteen days. Plug all valves that can be opened by livestock or fence the area around the well in order to prevent damage to property.
0869730780 2014-10-06 Production Wells No Violations On Monday September 29 I received a phone call from Mr. Dimmerling the land surface owner. Mr. Dimmerling stated that his cattle were eating the soil on the bank below the well. I tested the soil by mixing it with fresh water, and using a low range Quantab I determined the chlorides were at 148 parts per million. This result would indicate that no further soil remediation on the site is necessary.

1120171587 2015-11-09 Production Wells Violations Noted At 9:07 AM, I received a voice mail message from Joe Dimmerling. Mr. Dimmerling indicated that the well was again leaking oil. Upon arrival at 9:45 AM, I observed approximately one gallon of oil laying on the ground within a 24 of the wellhead. The source of the oil was a leaking polish rod liner packing. At 9:53 I spoke to Cory McCudden with Northwood Energy and notified him of the leak. I instructed him to remove all oil and any contaminated soil. He stated that he would dispatch personnel to clean and repair the well.

My inspection found the contaminated soil was removed and stone was placed in front of the pumping unit. All debris was removed and the wellhead was fenced off from livestock. The area still needs to be seeded.

1513900055 2017-12-18 Production Wells No Violations On 12/15/2017 at 3:02 PM I returned a call to land owner Joe Dimmerling. Mr. Dimmerling stated his concerns about a potential 3 foot by 3 foot area in the snow of what he believed to be brine that was released while a brine truck with Corp Energy written on the side was loading brine from the storage tank. On 12/15/17 at 3:20 PM I contacted Corp Energy Services, a contractor for Northwood Energy, and notified them of the potential release. During my inspection there was not any snow coverage and it was lightly raining. I could not locate any signs of a brine release. The well was equipped with 4.5 inch diameter production casing with a tubing head, 2.375 inch diameter tubing, rods, Trapper stuffing box leak detector, and a pumping unit powered by an electric motor. The production valves were in the open position. A line was attached to the drain port on the trapper and ran to a steel 55 gallon drum. The production equipment consists of one 100 barrel tank, one 210 barrel tank, one drip, a horizontal separator, and a meter run with a 2 inch diameter line, .375 inch orifice, and a chart recorder with a current chart. All production equipment is within an earthen dike. The identification was located on the tank and the production records were current.
1524488929 2018-04-20 Production Wells No Violations Joe Dimmerling called and left a voicemail at 11:40 a.m. on 4/20/18 stating that two small holes were found in one of the tanks that resembled bullet holes. During my inspection I was accompanied by land owner Joe Dimmerling. Two small puncture holes were found in the east side of the 210 barrel tank. The holes were slowly draining produced fluids. I called Mike Corp, a contractor for Northwood Energy, at 12:06 p.m. Mr. Corp directed a truck to come and remove the product from the 210 barrel tank. The tank was located within an earthen dike and there were not any fluids found on the outside of the dike.
1524746427 2018-04-20 Production Wells No Violations While inspecting well 3412123549 on the adjacent land Joe Dimmerling arrived on location. Mr. Dimmerling asked if I could check a spot near the lease road where he believes there was a brine release. When I arrived on location I could not find any obvious signs of a brine release in the area Mr. Dimmerling described. The area consisted of a dirt surface that was lightly graveled.
