0160572819 2017-05-09 Production Wells No Violations The purpose of my inspection was a status check on compliance notice 1333151288. The compliance notice issued to Green Gas Company in 2013 stated that the well was in violation of 1509.062 (A) for Nonproduction. During my inspection the well was equipped with 4.5 inch diameter production casing with a tubing head and 1.5 inch diameter tubing with a tubing valve. All valves were in the closed position, no production lines were attached to the well, and no production equipment was found. Since the year 2002 Green Gas has reported all zeros for production. No action has been taken to gain compliance and the original violation from the compliance notice 1333151288 still exist. Pictures were taken to assist this inspection.
0570593770 2015-09-24 Production Wells No Violations I am accompanied today by Inspector Sefcik, our inspection finds the well is shut in at the tubing and the casing. No pipelines are attached to the wellhead. No associated production equipment or tanks could be found. Photos were taken.
0605089612 2019-03-01 Production Wells No Violations The purpose of my inspection was to follow up on a previously issued compliance notice. On the day of my inspection I witnessed the well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing, with 1.5-inch diameter tubing. At the time of my inspection all valves were in the closed position, the 4.5-inch diameter valves were closed but did not have any plugs in the valves. The well was not tied to any production equipment and no production equipment could be located at the time of my inspection. I spoke with land owner John Foreman and he informed me that nothing had been done with this well for several years. There was no identification on the well and production has been reported as 0 since 2002.

There is no production equipment present. There has been no production reported since 2001. PLUG OR PRODUCE THE WELL. FAILURE TO COMPLY MAY RESULT FORFIETURE OF BOND.

1969531203 2015-09-29 Administrative Inspection No Violations At 2:29 pm I received a call from Scott Davis in response to a voice mail I left. Mr. Davis and I discussed the companys plans for the well. Mr. Davis stated that he was going plug the well. I reminded Mr. Davis that the well has been in violation for almost three years and that the company may face escalated enforcement action if the violations were not addressed quickly.
