1517603207 2018-02-02 Production Wells No Violations During my inspection the well was equipped 10 inch diameter conductor, 4.5 inch diameter production casing, and a 4.5 inch diameter to a 2 inch diameter swage nipple. Production valves were in the closed position. There were not any production lines attached to the well or any production equipment located. Identification was not located in a conspicuous place near the wellhead. Production records have not been submitted to ODNR. Land owners John Biedenbach and Stephen Biedenbach accompanied me while inspecting the well. The Biedenbachs stated that Jon Cleary currently owns the well.

1850567080 2017-07-18 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped 10 inch diameter conductor, 4.5 inch diameter production casing, a 4.5 inch diameter to a 2 inch diameter swage nipple, and an orange construction cone topping the well. All valves were in the closed position with no production lines attached to the wellhead and no production equipment on location. Identification was not located in a conspicuous place near the wellhead and no production records have been reported.
