0163082489 2018-01-23 Production Wells No Violations The purpose of my inspection was to follow up on compliance notice 1444753604. The well does not have any production equipment and does not have any production lines attached. No action has been taken to gain compliance and the original violations still exists. 0273591794 2016-05-02 Administrative Inspection No Violations My inspection today is an Administrative Inspection, purpose being a Compliance Notice Follow up. I spoke with Corey McCudden from Northwood Energy explaining to him that this well was given 180 days to reach compliance and those days have expired. Mr. McCudden stated that they successfully acquired a new lease with the land owner. Northwood Energy wants to get the well in production and tie in with well #3412121559. An email was sent to Mr. McCudden following the phone call conversation containing pictures of the well, the date the compliance notice was sent, and the date of the required action. Mr. McCudden will contact me when he obtains more information regarding this well.
0521860108 2016-04-25 Production Wells No Violations My inspection today is a follow up on a compliance notice 1444753604 that was sent to Northwood 10-13-15 for an idle well. The compliance notice stated well shall be plugged within 180 days. 180 days have expired. This well is equipped with 8.625 inch surface casing, 8.625 inch by 4.5 inch swage nipple, 4.5 inch casing head, and 1.5 inch tubing. The 1.5 inch tubing is rusted off at the casing head ring. Two 2 inch ports are located on both sides of the casing head. One port has a 2 inch steel line connected with a valve shut in. The other 2 inch port on the casing head has a 2 inch steel line that has rusted in two and the remaining portion of the steel line has fallen to the ground. Brine is seeping from the casing and causing the wellhead to rust away.
0653296642 2017-05-05 Production Wells No Violations The purpose of my inspection was to follow up on compliance notice 1444753604. The compliance notice stated violations 1509.12A defective casing and 1509.12B and the required action stated well shall be plugged. During my inspection the tubing head on the 4.5 inch diameter production casing had an open port leaving the production casing open to the atmosphere. The nipple and valve that was once connected to the tubing head port has rusted off and is laying on the ground. The 1.5 inch diameter tubing was left open ended leaving the tubing open to the atmosphere. Brine was seeping through each threaded portion on the well head leaving a puddle of brine on the surface where vegetation cannot grow. This well does not have any production equipment and does not have any production lines attached. No action has been taken to gain compliance and the original violation still exists.
1120780561 2015-11-30 Production Wells No Violations This inspection today is to follow up on a compliance notice. This is an idle well. The well was equipped with 8.625 inch casing into an 8.625 inch by 4.5 inch swage nipple into 4.5 inch casing. Threaded onto the 4.5 inch casing was a tubing head with two 2 inch nipples and valves coming off the sides of the tubing head. The valves were closed. There is no pumping unit on the well or any lines hooked up to the well. The well was seeping brine from the 8.625 inch and the 4.5 inch casing the well head is rusting away. The well is incapable of production. Since the compliance notice has been issued there have not been any changes to the well. Well shall be plugged. I will call Northwood Energy to discuss the plugging schedule.
1416809125 2016-05-02 Administrative Inspection No Violations I spoke with Corey McCudden from Northwood Energy discussing the compliance notice on this well. In October of 2015 a compliance notice was issued to Northwood energy for an idle well. Mr. McCudden stated that they just created a new lease for the well. The compliance notice stated the well shall be plugged within 180 days. Mr. McCudden is aware of the fact the 180 days have expired during April of 2016. No changes made, well is still idle.
1444753604 2015-10-13 Production Wells Violations Noted One the day of my inspection the well was equipped with 8.625 inch casing into an 8.625 inch by 4.5 inch swage nipple into 4.5 inch casing. Threaded onto the 4.5 inch casing was a tubing head with two 2 inch nipples and valves coming off the sides of the tubing head. The valves were closed. There is no pumping unit on the well or any lines hooked up to the well. The well was seeping brine from the 8.625 inch and the 4.5 inch casing the well head is rusting away. This well is incapable of production. The well owner/operator shall submit a written plugging schedule to Austin Gutridge at 5880 Memory Rd., Zanesville Ohio 43701 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
1501108809 2017-07-26 Production Wells No Violations The purpose of my inspection was to follow up on compliance notice 1444753604. The compliance notice stated violations 1509.12A defective casing and 1509.12B and the required action stated well shall be plugged. During my inspection the status of the well had not changed since the compliance notice had been issued. The original violation still exists. On 7/25/17 I received a phone call from Corey McCudden with Northwood Energy. Mr. McCudden stated that Northwood Energy is searching for an economically efficient sales line for the well to sale natural gas into.
1509722182 2017-11-02 Production Wells No Violations The purpose of my inspection was to follow up on compliance notice 1444753604. The compliance notice stated violations 1509.12 (A) defective casing and 1509.12 (B) well shall be plugged. During my inspection the status of the well has not changed since my last inspection. Brine was continuing to seep through each threaded portion on the well head leaving a puddle of brine at the base of the wellhead. The well does not have any production equipment and does not have any production lines attached. No action has been taken to gain compliance and the original violations still exists.
1541785126 2019-05-06 Production Wells No Violations The purpose of my inspection was a follow up on compliance notice 1444753604. The well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing, there was a tubing head installed on the 4.5-inch diameter production casing, the tubing was broken off at the tubing head, the well head was in disrepair and corroded. The well was open to the atmosphere and there was no fluid or gas that could be noticed flowing from the well. I will contact Bill Mitten and determine when they are planning on moving in to plug the well. Photographs were taken to assist with inspection.

Corey stated that Northwood was going to service the well and try to find an existing sales line to get the well hooked into.
