0501862924 2017-03-22 Orphan No Violations Orphan well status inspection, accompanied by DOGRM Geologist Jake Glascock: Upon inspection, it is observed that the condition of the well casing has deteriorated further since the last inspection. The 8.625 diameter surface casing and 1.5 diameter tubing are now bent-over, broken, and laid down on the ground surface. Salt crystals have precipitated on the well head, casing, and tubing from past brine leakage. The well is not currently leaking. Photos were taken of the well, former tank location, steel flow lines, and the access route.
1046103448 2015-09-16 Orphan No Violations My inspection today was to verify that the well is still leaking gas and brine and to confirm that nothing has changed. I found the well NOT leaking gas and water and that the area around the well had dried up. There are now holes rusted through the 8.62 diameter casing above ground level. The well was originally leaking above the wellhead but there were no natural gas oders detected today. The old pit/dike area where tanks had been set is still intact and needs filled in. Records show this well was drilled in 1972 to a depth of 5760 in the Clinton Formation. In 1974 this well was plugged back to the Ohio Shale. A plug was set across the Clinton Formation, then the 4.5 diameter casing was parted at 5020 and pulled. These records show that the well was produced through 1.5 diameter tubing, hung on the 8 diameter surface casing. This surface casing is set to a depth of 1584. Access to the well is across the adjoining (sisters) property, at 39.81769, -81.51736. New pictures taken.
1329171017 2012-02-13 Orphan No Violations Inspection revealed that well is equipped with 8.625 surface casing and 2 tubing set at unknown depth, records indicate that well was plugged back several years ago .Jim Brody landowner want I&O referral done on said well will start research as time permits.
1337605363 2012-05-21 Orphan No Violations Follow up inspection and initial start of I&O paperwork on said well , inspection found 8.625 surface casing and 2 tubing present both are in very poor condition with the tubing rusted off at the top of the well head and a very slight gas odor detected at the well head , inspector will note signs of what appeared to be the old tank battery on well location and a old 2 line sticking up out of the ground with a valve and plug that leads away from the well site that could of possibly been the old gas sales line ,area is holding water and will need reclaimed and line removed during the restoration process no other equipment is present related to said well ,inspector completed matrix -photographed well site -received signature from landowner on right of entry and completed drawing on well location on this day .
1343311422 2012-07-25 Orphan No Violations My inspection this day found idle/orphan well leaking small amount of brine. The 8 surface casing with swedge stands about 5 above ground level - 1.5 tubing can been seen in the swedge but has broken/rusted off flush with the swedge. Brine is trickling down the outside of 8 and a small kill area about 5-10 long below the well is caused from the brine - slight odor of gas. There is 2 steel coming off the well and also in the old tank dike area but is not connected. Pit still evident inside dike area wich is about 75 x 30 - will also need cleared of debris and reclaimed. ODNR records show 4.5 casing shot off and pulled - permit for a Berea recompletion. This is a landowner complaint.
