0976552996	2016-03-01	Production Wells	No Violations	I met with Ruth Burns the owner of RMB Corp. Ms. Burns stated that the well was plugged, but could not find any record of it. I walked the area around the coordinates in RBDMS and found no sign of a well. I was shown an indentation in the ground at 39.90483 -81.4444 by a former employee of RMB Corp. who stated that was the location of the well.
2103878712	2018-11-16	Production Wells	No Violations	On the day of my inspection I was unable to locate the well. After speaking with the landowner Bill Combs, he informed me that the well had been plugged years ago. There was no Plug Report on file in RBDMS. Southwest Supervisor Eric Conrad performed a field inspection in 2016 and was also unsuccessful at locating the well. Eric met with the owner of RMB Corporation Ruth Burns and according to his report she informed him that the well was plugged but she was unable to locate any documents regarding the plug job. He informed me that Ruth Burns the owner of RMB Corporation had passed away and there was no one operating the company anymore. I attempted to reach RMB Corporation with the contact information in RBDMS and was unsuccessful. After further investigation there are 5 wells that are listed in RBDMS as producing under the Bond.
