0801734746 2017-05-16 Orphan No Violations On location with SE Region Orphan Well Inspector Cindy VanDyke and DOGRM ER Coordinator Nathan Lane to perform temporary casing leak repair and contamination cleanup at this orphan well location. Upon digging down approximately 2 around the 5.1875 casing, a large split was observed in the casing with crude oil foaming out of it. Two 94 lb sacks of Type I Portland Cement were poured around the casing and mixed with freshwater to temporarily seal off the split in the casing. After pouring the cement, soil was piled up around the contaminated area to create a makeshift earthen dike. Finally, three bales of peat moss were spread over the free oil product to absorb and immobilize it until further cleanup is done before the well is plugged. Photos were taken of the work done to the orphan well location.

1495124865 2017-05-17 Orphan No Violations Due to the landowner, Aaron Reed, reporting oil around this well, L. J. Parkins, Nate Lane and I went out to clean-up the contaminated soil and repair the well. We dug down about 2 feet and found a cut in the casing about 3 inches long with gassy oil bubbling out of the cut. We dug below the cut, filled the surrounding hole in with 2 sacks of Sackrete mixed with water. We dug around the outer edges of the oily ground, forming a dike to prevent further contamination. We then spread 3 bales of peat moss over the oily area to soak up the free oil. We placed 6 steel posts in the ground and wrapped orange plastic fence around the posts, securing the fence with ties. L J. visited the site the next day and found the work as we had left it. The contaminated soil will be removed when the well is plugged. Pictures taken.
1519402979 2018-02-22 Orphan No Violations The purpose of my inspection was a status check on an orphan well. The land owner Arron Reed sent pictures to ODNR Division of Oil and Gas supervisor Eric Conrad of oil surrounding the well. During my inspection ODNRs Orphan well program had placed a boom to stop the flow of oil from entering into a nearby pond. The oil was contained at the base of the well. I spoke with ODNR Orphan well program administrator Gene Chini and he asked that I assist them by working with the Orphan inspector in removing the contamination. I will correlate with the Orphan program and schedule a time to assist them with the clean-up.
1680821221 2017-04-10 Orphan No Violations Orphan well status inspection in response to request from landowner Mr. Aaron Reed: Upon inspection, it is observed that the well is leaking oil from the subsurface portion of casing to the ground surface. The well is not visibly leaking above the ground surface. Oil is covering an area approximately 5 wide by 10 long and extending to the north from the wellhead location. Mr. Reed has stated that the well leaks oil every spring when the water table is high, however it has not leaked this much in the past. Photos were taken of the contaminated soil. Action will be taken to temporarily prevent the well from further leakage and to soak up/immobilize contamination on the ground surface.
1974293586 2017-05-17 Orphan No Violations The landowner, Aaron Reed, reported to the inspector that the well was now leaking oil. The cows were getting into the oil around the well which covered a 5-foot circle of ground was heading downhill. L J Parkins and Nate Lane accompanied me to help clean up the oily area and put up a fence to prevent the cows from entering. We dug around the casing 2 feet below ground level where we found the casing had been bent over and leaking from a horizontal cut in the pipe. We placed 2 sacks of Sackrete around the casing to seal up the hole and added water to harden the cement. We also dug around the contaminated ground and diked it up to prevent further leakage. Three bags of peat moss were spread to cover the oily area. Then 6 steel posts were drove in the ground and orange plastic fence was placed around the posts and tied in place, to prevent the cows from spreading the contaminated soil. L J Parkins checked the next day on the location and found the well was not leaking.
2142541667 2014-03-06 Orphan No Violations My inspection this day needed to gather information to enter the well into the orphan program. The well consists of 5.25 OD casing with a wellhead with 2 ports that are shut in. There has been a valve and gauge on it in the past. The wellhead is showing signs of leaking oil but only in small amount and probably occurs with a warm-up of the weather. This well is 248 deep and drilled in 1953. Pictures taken, SEM is 95, verbal complaint by landowner.
