0871048876 2019-04-02 Production Wells No Violations The purpose of my inspection was to follow up on a phone call on 4/1/2019 from Scott King who said that an excavator had struck a well head and the well was releasing gas. I was out of service area at the time of the call. At the time of my inspection the well was still releasing gas the well was equipped with 5.5-inch diameter production casing and 2.375-inch diameter tubing, the casing had a 2-port style well head with a packer system on the tubing. The 5.5-inch diameter casing appeared to be shut in utilizing a 1-inch diameter line and 1-inch diameter valve off one side of the well head and a 2-inch diameter flat plug off the other side of the well head. The 2.375-inch diameter tubing was running through a 2-inch diameter Tee with one side attached to a 2-inch diameter steel flow line with a 2-inch diameter valve that was closed, the top of the Tee had been attached to a 2-inch diameter nipple that went to a 2-inch diameter variable close valve that had been broken off. When I arrived Nate Lane with Emergency Response and Cinthia Van Dyke orphan well inspector had just left the well and Cinthia informed me that she was performing the orphan well inspection, and would contact a contractor to get the well under control and shut in.
