0098730752 2015-03-10 Plug / Plug Back No Violations This day, I arrived on location at 9 AM, along with track hoe operator and his crew. Charlie (Buckeye Water Service employee) arrived at 11 AM after gathering up parts and equipment. A dump truck brought a load of gravel and spread on the private driveway we were using to access the well. The track hoe dug around and removed contaminated soil from around the casing and placed it in a roll-away container. A 4 tall, 8 wide steel culvert acting as a cellar was placed in the hole around the well and 10 sacks of Quickrete were added to stabilize and seal off the bottom of the cellar. Next, timbers were placed across the top of the cellar to build a platform from which the crew will work. Charlie plans to finish building the platform and set the rig up over the hole yet today.
0787527700 2015-06-15 Plug / Plug Back No Violations Site inspection this day found the location has been disked, smoothed and seeded. It has been mulched heavily with straw and is very uneven, thick in spots and almost missing in others, but I can see the grass seed is sprouting. I found all the equipment has been removed and the location is passable for the Final Restoration. I will speak with the landowner, Ed. Crock and get him to sign a waiver that he his satisfied which will complete this process.
1156710705 2015-03-09 Plug / Plug Back No Violations I arrived on location to find the steel tank and cellar that goes around the well casing to prevent spills, had been unloaded. The rig and dozer are already on site. The track hoe crew arrived and will bring the track hoe tomorrow and will get cellar installed, rig and other equipment set in place.
1215442765 2015-06-29 Final Restoration No Violations A final restoration inspection was conducted today and found that all equipment has been removed from the site, vegetative cover is established, the landowner is satisfied and has signed the Landowner Waiver. The site Restoration is found to be satisfactory and Final Restoration is passed.
1336576227 2012-05-09 Orphan No Violations Visited with property owner -Edward Crock and received signature on right of entry form on I&O well on his property ,Mr. Crock asked for complete copy of all records once plugging and restoration is complete .Well is equipped with casing but it is not visible at the time of inspection ,it also has 2 tubing and rods ,well is leaking oil and has affected area around well head that is in an area prone to flooding and in close proximity to a stream .I&O packet started .
1390311685 2014-01-16 Orphan No Violations My inspection this day with Inspector Eric Conrad, is needed to update and enter this well into the Orphan Well Program. The well was drilled in 1942 to 245 TD. Casing was not visible but suspect 6 production casing. There is a large area 15 x 10 around the well that is saturated with oil and stained orangish from brine saturation. The well lies 30 uphill from Salt Run and there is no brine or oil entering the creek. This well should be easy to plug and there may be others in the area that could be put with it to make a project. SEM is 195, pictures taken.
1404159781 2015-06-01 Plug / Plug Back No Violations My inspection this day was to sign off on the restoration work for the well location but find it has still not been done. There are still some shallow ruts and several patches of ground which need seeded, mulched and fertilized. I talked to the adjacent landowner, who maintains the Crock property, and he said he had been working to improve the area. He stated that the drainage ditch is now deeper than it used to be and that there is too much gravel on the access road, making it harder for him to mow. I told him we would correct these issues as well as those on the well site.
1426166415 2015-03-11 Plug / Plug Back No Violations After completing the rig set-up, rods were pulled along with the tubing (rods would not come free), unscrewing the tubing and cutting the rods with a saw. Total tubing was 240.5. They then ran tools in the hole and found bottom to be 247. A 2nd steel tank for fluids was brought out on location and the cellar pumped into it. The formation water is unusually high in chlorides but the hole in making very little fluid or gas. Cementing planned for tomorrow.
1426542590 2015-03-16 Plug / Plug Back No Violations When I arrived today, the crew was already loading up the equipment to move to the Anderson (#121-2-3002) location. The cellar was removed and loaded and the dozer worked at doing a rough grade on the location. The cellar area around the well is still open and waiting for a load of topsoil and the half tank containing contaminated soil needs to be removed.

1434402623 2015-06-15 Final Restoration No Violations The location has been disced to smooth, seeded and mulched with straw. Grass is starting to grow. All equipment has been removed. This site passes Final Restoration.
1567579420 2014-01-16 Orphan No Violations My inspection today finds the well is equipped with 6 production casing, 2 tubing, and rods. There is a 15 x 10 area around the well that is saturated with oil and stained orange by brine. I am accompanied by inspector Cindy Van Dyke.
2111542268 2015-05-18 Final Restoration No Violations My inspection today finds the well site does not meet the final restoration requirements. The well site needs to be graded and vegetation needs to be established. I notified Gene Chini of the deficiencies in the well site.
