1000953846 2016-03-03 Orphan No Violations I received a telephone call from Don Warnock, the Antrim VFD Chief, indicating that he received a call and a video of this well from a Ms. Janet Muffet. Mr. Warnock emailed the video to me. I drove to location where I met Mr. Warnock. I inspected the well and found the well was equipped with 1 tubing that was open to atmosphere. The well was not flowing as depicted in the video at the time of inspection. Mr. Warnock stated that he was told the video was shot the day before (3/2/16). Orphan well inspector Cynthia Van Dyke met us on location. A low range quantab was used to test the water in the stream at the point where water from the well may have entered based on the discoloration of the soil on the bank. The test returned a chloride level that was below the Quantabs readable range. No indication of hydrocarbons were noted in the stream or on soil around well. I called Trevor Irwin of the OEPA and informed him that well was now static and any impact to the stream that may have occurred appears to have been minimal.

1397839902 2014-04-01 Orphan No Violations My inspection this day to update information provided by previous landowner, W. Smierciak, and Inspector B. Stolarik. This was initiated by a complaint by Eric Wilkens, a neighbor, stating this well was abandoned and leaking brine and gas. I found the well equipped with 1.5 diameter tubing standing 3 above groundlevel. This tubing was equipped with a valve that is attached to 1.25 OD tubing that goies below ground. There is no evidence of a wellhead or casing but I heard gas bubbling below ground. An 18 x 36 kill area exists downgradient from the well and extends into Depue Run which is located on the west side of the well. All fittings, piping, the separator (150 gallon) and drip are very rusted and the ground in the kill area stained orange. A high Quantab tested off the scale, indicating chloride levels exceeding 6000 ppm. There is orange staining on some of the surrounding brush and tree branches indicating that the well has unloaded in the past. There was no petroleum odor detected at the site. Available records indicate thet this well was drilled in 9/39 to the Berea sand and was plugged in 11/39. These records show that this well is making gas - filled to top with sand laden fluid - tubing left in. Pictures taken, SEM is 190.
1446579913 2015-11-03 Orphan No Violations My inspection today was needed to determine if there was surface casing, or any casing, in the well. I dug around the 1.5 tubing on both pieces - they are each going underground about 2 apart - digging about 1 deep. I discovered that the east one had gas bubbles coming around the outside of the tubing and about 1 out from the tubing. The west piece of tubing had no bubbles, thus I believe the east string is where the hole is. I probed about 3 down after digging it out and stirred up no bubblies on the west tubing but considerable bubbles on the east one. There is a slight odor of gas around the well. The brine pool around the tubing went down the east wellbore after probing enough and the holes where the tubing entered the subsurface dried up over the next few minutes. There was an obvious void around the east tubing but I did not feel any casing on either hole. The void would be harder to explain if there was no casing in the well. The water draining through the void section had a different sound, making a sucking sound. The plugging report from 1939 says they left the tubing in and filled the hole with fluid-laden sand. Much of what I dug up was sand. No mention what they did with other casing. New pictures taken.
1457096766 2016-03-03 Orphan No Violations DOGRM Noble Co. DOGRM Inspector Eric Conrad was contacted by Don Warnock, Antrim VFD Chief, in response to this well open flowing water and gas into the environment. Inspector Conrad reveived a video filmed the previous day, 3/2/16, showing the well unloading and has forwarded it to our office. This day, Inspector Conrad and I wet with Mr. Warnock on location. We found the well static (not flowing) as I have observed it during prior site inspections. There is a large (20 x 25) scald area on the ground around the well and staining on surrounding tree branches which verifies that the well periodically unloads. The scald area extends down to the edge of DePue Run. The stream water was tested for chlorides using a low range Quantab at the point where the wells discharge appeared to enter the stream. No chlorides were detected. There was no visual sign of gas coming out of the open 1 diameter tubing today but previous inspections found gas being emitted around the tubing below grade after some of the dirt around the tubing was hand excavated. New pictures were taken today.
1457446065 2016-03-04 Orphan No Violations Bob Lane, contractor for the Orphan Well Program and I arrived on site today to further evaluate this well. We hand excavated around the tubings to a depth of 2.5-3 feet. We found that gas was not bubbling out around the tubing and that the tubing that was below ground level was in good shape, as compared to the above ground piping and equipment that was severely corroded. The tubing to the east was found to be the actual hole and while we could not dig down to expose it, we realized there was additional casing in the well, based on how the hole acted and dug out. The other tubing to the west was also dug around and found to be the sales line leaving the well. The tubing went underground and turned to continue westward. There was also found additional 3 steel line, partially buried south of the well and additional 1 steel line trending southwest of the well. Today the well was not unloading and there was no gas seen coming from the open 1 tubing in the hole. Plans were made to bring a small excavator on site and dig further down around the well. The 1 tubing can then be sealed up with a valve to stop further unloading of gas and water and the casing can be exposed and evaluated.
1495124950 2017-03-22 Orphan No Violations Orphan well status inspection, accompanied by DOGRM Geologist Jake Glascock: Upon inspection, it is observed that this well is idle, orphan, and has created significant surface impact from brine release. The well is not leaking this day, however brine impacted soils extend from the well to the nearby stream bank (25-30 west). The 2 diameter tubing is broken off below a tee fitting near its top. No other casing is visible at the surface. There is a telephone line down in the middle of the access route to the well which will need repaired before mobilizing to plug. Photos were taken.
