1036274184 2018-09-19 Plug / Plug Back No Violations The inspection is conducted under plugging operations. Steve Miner called on 9-17-- inch tubing was run to 3000 feet. The Clinton/ Medina has caved in and will not allow the tubing to be ran further than 3000 feet. The lime plug was set a little high and there was an additional 100 feet added to the cement plug with 2% gel and 2% calcium chloride with cotton seed hulls. The Big Lime plug was set from 3000 feet to 2700 feet. Formation Cementing set a 300-foot plug with 75-sacks of Class A cement @ 2% gel and 2%CaCl with cotton seed hulls and then displaced down with 10.6 barrels of salt water for the displacement. The well was pumped on to try get circulation established. The bottom of the surface plug was set from 1768 feet to 1468 feet. Formation Cementing set plug of 78 sacks of regular class A cement 2%gel and 2%CaCl with cotton seed hulls thru 2 3/8- inch tubing. The displacement was 5.6 barrels of salt water. The well had still not circulated at this time. The decision was made to shut down and come back next day on 9-20-18.

The rig ran 2 3/8--6829 feet and Trenton was at 6566- 6570 feet. Formation Cementing services ran 40 sacks of Regular Class A cement with 15 lbs. of cello flake and set plug from 6826- 6326 feet. The displacement was at 25.6 barrels of 10 lb. per gallon salt water. Circulation was established.
1878772887 2018-09-20 Plug / Plug Back No Violations The inspection was conducted under plugging operations. Bishop well services rig had run 2 3/8 inch down and tagged the top of cement at 1732 feet. Jay Hawk water truck loaded the casing. Formation Cementing pumped all salt water out of the 8 5/8 casing after breaking circulation. The cement plug was then placed from 1732 feet to 1532 feet at the bottom of the surface. Formation Cementing placed a 62 sack of regular class A cement at 2% Calcium Chloride. Bishop pulled up to 200 and Formation Cementing circulated the last plug to surface. They ran 64 sacks of regular class A cement. The cement slurry did not drop back.
