0585961573 2018-04-24 Production Wells No Violations The inspection is conducted under rainy conditions and 52 degrees Fahrenheit and is a Compliance Notice follow-up to number -1612530905 and complaint number 5621. Brian Bilotovich accompanied me on the inspection and revealed the well is equipped with a pumping unit, tubing head and 2 3/8- inch tubing. There is mess at the base of the tubing head. The valves are open. The belts are burnt off the electric motor on the unit. The tank site consists of [1] 210-barrel steel tank and horizontal production unit with a chart meter. The line pressure is 200 psi and the differential pressure on the two-pin chart is 50 psi. The electric line still needs to be reattached to the power poles.

There is an impoundment in front of the unit with water in it that needs removed. The chart meter shows no production at the horizontal separator with no pressure. The [1]210- barrel steel tank is rough with a spot on the side that looks wet. The tank has no earthen impoundment that enclosed the tank. The identification is posted at the tank.
1431467876 2016-05-27 Production Wells No Violations On the day of my inspection I met with Bill Magraw of Petrox on the well issues. The culvert needs fixed and remove the old culvert. I stated no more venting of the gas. Cleanup the contamination and remove it. He will get started on the work as soon as it dries out.
1612530905 2017-06-05 Production Wells Violations Noted On day of my Inspection Brent Bear and I met with the Complainant on the well due to Complaint number 5621. The following was determined: The well is producing and cycling on the day of the inspection. The well is equipped with Pumping unit and tubing head. The production manifold is open at the rod and tubing annulus. There is (1) 210 barrel steel tank and a horizontal separator with a chart meter that is working. The identification is posted on the tank. There will need to be an impoundment constructed that will enclose the tank and the production equipment. The pumping unit is supplied with an electric motor for the prime mover. The electric line is pinched down in several places and need debris removed and the line pulled up. There is a small impoundment in front of the wellhead that needs cleanup and hauled away to an approved landfill. The manifests will need to be provided to the inspector. The old culvert pipe needs to be hauled away.
1617898593 2016-04-21 Production Wells No Violations Accompanied by inspector, Pat Shreve, our inspection today is a complaint based status check on a producing well. The well is equipped with 8.625 inch surface casing, 4.5 inch production casing, 2 inch tubing, rods, and a pumping unit. The tubing valve is opened. The 4.5 inch production casing is venting gas through a pop off valve. There is a small oil patch around the well head. The production equipment included one 210 bbl. tank and a horizontal separator. Proper identification is located on the tank. There is extensive erosion in the portion of the lease road that has a creek under passing. Pictures were taken of the lease road erosion and oil patch around well head. Inspector Pat Shreve will be contacting Petrox to work with them on fixing the violations.
1633746310 2019-05-07 Production Wells Violations Noted The inspection was due to a Complaint filed under number 6153. I was accompanied by Emergency response officer Jason Blocker of ODNR. Ohio EPA officer on site was Aaron Wolfe and Landowners Brian and Laura Biolovich. The well is equipped with a pumping unit, electric motor and no belts, 4 1/2inch casing, tubing head and 2 3/8-inch tubing. The tubing and casing valves are open. The tank site consists of [1]-210-barrel steel tank and 1 horizontal separator with a non- working chart meter. The pipeline is open and going towards the PN 5110 well. Approximately 150 feet west is the 2-inch pipeline which a tree had fell across and pierced the line. This caused approximately 20 gallons of crude to exit and enter an unnamed intermittent stream. The company had responded to the call that was placed the night before and was on site cleaning crude oil up with absorbent pads. The well is shut in and the gas has vented out. The oil has been contained and will monitored and clean-ed up. The company has five days to clean up the issues
1694515992 2017-12-13 Production Wells No Violations The purpose of my inspection is due to a Compliance Notice Follow-up to number -1612530905 based on complaint number 5621. The well was cycling on the day of my inspection. The well has an impoundment in front of the unit with oil contaminated soil that has not been removed and hauled away. The [1]210 barrel steel tank is bubbling up down the side and looks like it could be leaking from the top. The culvert pipe pieces were not hauled away. None of the violations were corrected on the day of the inspection.
