The identification is posted on the tank. There will need to be an impoundment constructed that will enclose the tank and the production equipment. The pumping unit is supplied with an electric motor for the prime mover. The electric line is pinched down in several places and need debris removed and the line pulled up. There is a small impoundment in front of the wellhead that needs cleanup and hauled away to an approved landfill. The manifests will need to be provided to the inspector. The old culvert pipe needs to be hauled away. |

The well is equipped with 8.625 inch surface casing, 4.5 inch production casing, 2 inch tubing, rods, and a pumping unit. The tubing valve is opened. The 4.5 inch production casing is venting gas through a pop off valve. There is a small oil patch around the well head. The production equipment included one 210 bbl. tank and a horizontal separator. Proper identification is located on the tank. There is extensive erosion in the portion of the lease road that has a creek under passing. Pictures were taken of the lease road erosion and oil patch around well head. Inspector Pat Shreve will be contacting Petrox to work with them on fixing the violations. |

Ohio EPA officer on site was Aaron Wolfe and Landowners Brian and Laura Biolovich. The well is equipped with a pumping unit, electric motor and no belts, 4 1/2inch casing, tubing head and 2 3/8-inch tubing. The tubing and casing valves are open. The tank site consists of [1]-210-barrel steel tank and 1 horizontal separator with a non- working chart meter. The pipeline is open and going towards the PN 5110 well. Approximately 150 feet west is the 2-inch pipeline which a tree had fell across and pierced the line. This caused approximately 20 gallons of crude to exit and enter an unnamed intermittent stream. The company had responded to the call that was placed the night before and was on site cleaning crude oil up with absorbent pads. The well is shut in and the gas has vented out. The oil has been contained and will monitored and clean-ed up. The company has five days to clean up the issues |
