0207921182 2014-02-03 Orphan No Violations On the day of my inspection I visited the site with Muskingham County Inspector Pat Shreve and Orphan Well Inspector Brent Bear. The exposed casing was callipered at 8.625 and it was open to the atmosphere. The line ran out at around 700 when attempting to measure depth. The fluid level was estimated to be around 175 to 200. There was CH4 detected at this well.
0419235900 2014-02-03 Orphan No Violations An inspection was conducted with Pat Shreve and Chris Grimm to verify the total depth of this well with a heavier weight. A depth of 712 was reached before running out of available line attached to the weight. During the retrieval, it was noted that this weight contained paraffin. The wellbore was monitored with a gas meter and showed the following maximum levels that were reached: LEL= 15 and CH4= 1.3. The fluid level inside this wellbore was estimated between 175 - 200.
0481338588 2012-01-06 Final Restoration No Violations The well is plugged, the pit has been back field, the tank and all other production equipment has been removed, there is no vegetation where the pit was, no erosion problem.
0496539030 2013-03-26 Orphan No Violations Well needs to be put on orphan status. The well is a stable 8.625 casing at surface and fluid is down 600. Well may have been plugged on affidavit. The casing was never removed. The plugs have sled down the well. Brent Bear and I went in and took pictures and established an entrance point.
0579383554 2012-01-26 Final Restoration No Violations On the day of my inspection I found the well was not plugged, there is (8) inch casing standing out of the ground with no cap, there is no production equipment and no pit.
0602699066 2013-04-26 Orphan No Violations A follow-up inspection revealed a change to the status of wellbore since the last inspection. A depth counter equipped with a braided line and a 2 oz sinker was utilized for this inspection to gauge the fluid level and wellbore depth inside the open 8.63 casing. Note: Wellbore was located underneath a 55 gallon drum. The fluid level was measured at 100 and total depth was found at 136. The material encountered at 136 resembled a soft clay-like material. An intermittent gas rumble could be heard within the wellbore and was also detected with a gas meter to show the following readings: LEL= 3; CH4= .10; and O2= 20.9. During the retrieval of equipment from the wellbore, a visual inspection of the line and sinker showed a reddish brown colored film with a crude oil smell and feel. The property contained an existing lease road with substantial erosion that had ruts which measured from approx 6 to 2 from mid-section to bottom of road. A GPS reading for this lease road was recorded from this inspection.

1353042352 2012-11-15 Production Wells No Violations Well is was supposed to be plugged in 1975. The 8.625casing is sticking out of the ground with 55 gallon drum over it. The fluid is down 500. Old lease road still somewhat in place but washing out. I will work with Cindy Vandyke and get this on Idle Orphan list.
1366379159 2013-03-26 Orphan No Violations An inspection conducted Mineral Resources Inspector 3, Mr. Pat Shreve, revealed an open 8.625 casing, in good condition, with no wellhead. The wellbore was not leaking fluids and natural gas could not be smelled or detected with a gas meter at the surface. The fluid level inside this casing was found at 12 seconds downhole from the surface. The property contained oilfield debris that consisted of steel line(s), and a 17 diameter casing attached with a steel line on bottom that resembled a setup for a dump ditch on a cable tool rig. A lease road was also observed on this property and found to contain erosion that measured 6- 1 from the mid-section to bottom of road. A GPS reading was obtained and recorded for the entrance into the lease road.

She further stated that the attorney over the estate would need to review the documents.
