On the day of my inspection Paul Carder with (RSS) Radiological Safety Section, (EAS) Environmental Assessment Section, and I were on site. Knox Energy Inc. was on site with a large track-hoe and a toothless bucket, and a skid steer. Jeff Berry with Division of Wildlife showed up and took a report. Knox Energy Inc. started digging down and reaching the bottom valve on the 100 barrel steel tank. The oil saturated soils were hauled to roll--off box delivered and the 4th box in the morning. At approximately 12:00 pm the (OEPA) Ohio Environmental Protection Agency showed up with solid orange hard booms. The contractor placed the orange booms at the overflow pipe on the 1st pond and the entry point on the 2nd pond. The contractor was removing and hauling oil saturated soils all day. The (RSS) was taking a sample from each bucket load. Please refer to (EAS) and (RSS) for their information. |

He stated the landowner that called in was named Mike Nasser at 9600 Matchett road. I arrived at approximately 17:15 and met with Mike Nasser the complainant. I proceeded taking pictures and found that the crude oil had leaked from the bottom of a [[1]100 barrel steel tank on the site. The release appeared to be from the tank bottom. I stated to Mr. Barr that he needed to clean up the site and remove the bad tank. He stated he was in Bankruptcy with Big Sky Energy and has no money to have the work completed. Mr. Barr stated he would send Greg Henderson, his son in-law out in morning and per my request no later than 9-1 |

Big Sky Energy windrowed the oil saturated soils and started placing the oil saturated soils at the base of the tank. I asked the company if they were going to remove the soils. They stated that they would have to get larger equipment in to perform this task. I stated that the soils would have to be hauled out and taken to a landfill. Mr. Barr stated that he will get with his Nephew Ronny Barr, and call me later this evening. At approximately 21:02 I called Bob Barr of Big Sky Energy and left a message. He texted me back at 11:00 and stated that they have sucked the oil off the trailer and tightened up the tracks on the Bobcat. Mr. Barr stated that if Greg Henderson, his son-in-law is willing to help, then they could haul the soils with a dump trailer. Mr. Barr is thinking about 4-5 loads with the dump trailer and will start around 10 am. I stated by a text message to Mr. Barr that we need to talk in the morning about the testing of solids and tank for (TENORM) Technologically Enhanced Naturally Occurring Radioactive Material. I will talk to Mr. Barr in the Morning at 8:00am. |

The track hoe was in the disturbed area and was removing the oil saturated soils. The skid steer would load up and (RSS) would take a sample from each bucket. The water truck was on the gravel and ran hoses across hollow to vacuum water from holes dug to find origins and ending points on the oil under- ground. The first hole up by the tank was dug and is noted as Exhibit G and it measured 4-foot-deep by 21 foot in length and the width of 3 foot. There was only a small piece of 2-inch pipe about 4 feet long. The hole was covered back up due to no oil at this hole. The second hole dug is down just south of the pond noted as Exhibit A or oil pit and it measured 4-foot x 8-foot x 4 foot deep. The oil was migrating from a piece of 4-inch drain tile and into Exhibit A. The third hole that was dug was at Exhibit D to Exhibit F. The water truck was flushing, fresh water down at Exhibit D with a dye and found that the dye was coming down to Exhibit A. The oil and sheen were being cleaned up with a flush and suction method. |

King Excavating showed up to the site with a Track--off boxes. The two trucks loaded up the two boxes and would mobilize them to Kilbarger landfill located in Logan, Oh. The two trucks would come back and get the remaining boxes out. Pictures were taken before the final grade was in--steer. Knox Energy spread fertilizer, seed and mulched in place. Knox Energy pulled all booms from the first pond. King Excavating graded road and entry point. All work was completed by 4:00 pm on 4-11-18. I will be back to see that proper vegetation has been established at a later date. |
