0403519737 2013-05-08 Orphan No Violations An inspection revealed a 32 diameter hole with fluid and soft clay-like material near the surface. There was no observed leakage of fluid or natural gas at the surface. A metal detector utilized for this inspection was placed directly over the hole and indicated the possibility of casing beneath the fluid. Below this location, in a wooded area, an oval shaped pit measured approximately 25 x 25 with a wall height of 10 - 11 on one side. The pit contained mature trees along the outside and oil saturated ground surface throughout the inside of pit. The overall condition of the oil was old/weathered from the surface to a depth of approximately 1. High Range Quantabs were placed in the fluid and showed chloride (Cl-) level below the US EPA recommended aesthetic value of 250 ppm (mg/L) Cl-. A potential source for contamination could have been a 2 steel line that derived from the well location approx 30 away and was routed alongside this location. Above the pit, a section of this line was suspended in the air/bent above ground with staining underneath the line at the collar. The line tees at the well and contained a threaded cap on the end. Additional oilfield debris on the property consisted of 1 joint of 7 casing and steel cable.
0819231280 2012-10-23 Orphan No Violations Cindy Vandyke and I inspected site and old pit that was used during drilling. We took updated pictures and the old hole is subsiding more on each inspection. We stopped by house at winery to get right away of entry signed. No one was there I will talk to landowner Diane Jaynes. Complaint # is 3963.
1148592648 2019-03-25 Orphan No Violations I witnessed Mark Hines with MH Oilfield clean up debris and contaminated soil associated with this well. The contaminated soil was excavated from the pit and hauled to a landfill. The steel flow line was removed from the property. I observed Mark Hines dig down where the well was located with a mini-hoe. Mr. Hines dug down approximately 5 feet and did not find any casing. Mr. Hines did appear to be in the wellbore as we observed clay where the well was believed to be located. Mr. Hines filled half the hole with 20 bags of cement and placed a steel identification plate on top of the cement. The plate and cement was then backfilled with dirt. Several photos were taken of the work completed. Time of inspection: 8:20 AM.
1347273198 2012-08-30 Orphan No Violations Met with Jack Green of Oxford and son of landowner Ben Jaynes. The well has been plugged due to lost hole . Oxford did not own this well at all. Oxford met with landowner and I for possible help on closure of old pit and old well bore. The old well bore is starting to subside.

1920927728 2012-10-23 Orphan No Violations My inspection this day with Pat Shreve to begin I/O process for this complaint from Mrs. Jahnes. She has found open hole with no casing, hole being 16-18 across and 18-24 deep - could be a hazard that a person walking could fall into and get injured. Nearby leaning against the tree is 2 fittings and about 30 away is what looks to be an old pit or dike area that is filled with black tar or oil and very spongy. The hole should be excavated to determine if a well that surface plug has dropped or if well was never plugged - records show 360 deep and plugged in 1959 as a lost hole. Dike area containing oily mess is 20 x 20 square and 2 old small steel drums are laying in the area. Dirt should be removed and contaminated soil hauled away, new dirt brought in, drums salvaged along with 2 steel pipe running from the well to the dike area on top of the ground. Access to the well is at N40.00674, W82.15535 where a culvert has been installed. Overhead electric powerlines should cause no problem accessing the well. Landowner not home this day but Pat will get ROE signed. SEM is 177, pictures taken.
