0108316965 2015-07-21 Production Wells No Violations I was contacted by Artex letting me know they would be plugging this well on 07/23/2015. We went over the plugging of the well during the conversation.
0588692926 2014-06-04 Drill / Deepen / Reopen No Violations Roustabout crew has graded location and hooked up production equipment. Well is equipped with a pumpjack however the well is not currently in production. There is one 210 bbl tank in the dike and a separator. Site does not meet preliminary restoration requirements. I will continue to monitor their progress.

1043590684 2015-07-23 Plug / Plug Back No Violations I was at this well today to witness the bottom plug being set. Bishop Well Service has moved there service rig in on the well to plug it. Formation Cementing was on site to set the bottom plug. We were able to get circulation on the well after pumping 20bbl down hole. After the well was cleaned up we then set a 400ft bottom plug. 30sks Class A cement was used. The rig crew will be pulling the tubing out of the hole today and maybe trying to rip the 4.5.

1217640055 2015-07-28 Plug / Plug Back No Violations Bishop Well Service was on location today ripping the 7. They were able to get ripped at 2,184. The 7 will be pulled then the tubing will be ran back in the well to the rip to set a rip plug.
1339811177 2014-07-22 Drill / Deepen / Reopen No Violations The pumpjack was removed from location. The valves on wellhead and separator are open, and the valve is open at the transmission line connection. There is a meter with a blank chart in the separator. There are new straw bales on location.
1376807812 2013-08-18 Casing String Witnessed No Violations I was on location to witness the cementing of the 9 5/8 surface casing. The Kilbarger IDECO Rig #17 drilled a 12 1/4 hole to a TD of 754. The casing shoe depth was 706. Centralizers were not run on the surface casing and the baffle insert was at a depth of 673. Formation cement on location at 4:00am. The plug was down at 850am with good cement returns, and casing pressure was held at 700 psi for 5 minutes.
1438019002 2015-07-27 Plug / Plug Back No Violations I was at this well to witness the plugging of it. Bishop well services was able to rip the 4.5 at 3,759. We made sure the pipe was free then had Formation Cementing set 150ft rip plug. The 4.5 was then pulled up to the bottom of the 7 to set the next plug. The Clinton formation was behind the 4.5 so the rip plug covered it. The top of the Big Lime is behind the 7 and is cemented. We set a 100ft in 100ft out at the bottom of the 7 for a 200ft plug. The 4.5 will be pulled from the well so they an try to retrieve some of the 7.
1463147032 2016-05-13 Final Restoration No Violations My inspection today is a status check on a final restoration. During my inspection I witnessed no evidence of an oil and gas well and the site has been restored. Location has met 1509.072 B requirements.
1914102063 2015-07-29 Plug / Plug Back No Violations I am on site to witness the plugging of this well today. Bishop has ran the tubing back in the well to set rip plug, the Berea, bottom of the surface and the surface plug. The surface plug did not stay at the surface so we shut down for the day to see where it settles at.
1986815326 2015-07-24 Plug / Plug Back No Violations Cement plugging for artex Oil. The bottom hole plug was redone today as the plug job on Thursday failed. The tubing was found to contain cement. It was determined there most likely a hole in the tubing allowing the well to maintain circulation. New tubing was brought out today and ran in the hole. Formation Cementing on site at 9:43 AM. The plug circulated.
