0168104051 2013-07-24 Casing String Witnessed No Violations I was on location to witness the cementing of the 4.5 production casing. The TVD of the well was 6400 and the casing shoe was at 6395. Universal Well Services were on lcoation and set 97 sacks of Class A/Type 1 cement.

0315366260 2015-08-06 Plug / Plug Back No Violations I was on location to finish out the plug job. We tagged the Berea plug first thin and it had slipped down hole 250ft so we ran tubing back to the Berea to reset the plug. After we pumped the Berea plug we waited an hour for cement to set up then established circulation again then pumped the bottom of the surface plug. We then pulled the tubing up to set the 200ft surface plug. The cement stayed at the surface. The well is now plugged.

Formation Cementing arrived on site to pump on the well. We established circulation after 20bbl then pumped the 400ft plug to bottom using Class A cement.

1255740208 2013-07-09 Casing String Witnessed No Violations I was called to location to witness the cementing of the surface casing by Formation Cement CO at 430am. Due to heavy rain and flooding cementers arrived around 500am and the location was in very rough condition. Artex is using the Kilbarger Patterson #17 rig. The Kilbarger drilled a 12 1/4 hole to a TD of 740. Artex had miscalculated the casing and had to send of for a landing joint from Miller supply. They sent cementers and inspector off location. We arrived back on location around 900am and due to the thick amounts of mud and clay at the location, the cement truck ended up stuck for some time until a dozer could get it to proper spot for cement job. The casing shoe depth was 689. Centralizers were not run on the surface casing and the baffle insert was at a depth of 646. The plug was down at 12:30pm with good cement returns and pressure was held at 500psi for 5 minutes.
1372855519 2013-07-03 Drill / Deepen / Reopen No Violations I arrived, after only given a 25 minute notice, to see cement trucks leaving location. Artex is using the Kilbarger Patterson #17 rig and Formation Cement Co. The hole was drilled to 90 and the conductor was set at 84. Heath told me it was a good cement job with good returns.
1373367730 2013-07-09 Drill / Deepen / Reopen No Violations I was called to location in order to witness the cementing of the surface casing. There were multiple areas of flash flooding in and around the area. Artex had finished placing casing in the hole upon arrival, however when the cement trucks arrived it was determined by the operator that they had miscalculated casing by about 14 and needed to obtain a pup joint from Miller Supply. Artex then told us the cement job will not happen for another 4-5 hours.
1438695541 2015-08-04 Plug / Plug Back No Violations I was onsite to witness the rip plug for the 4.5 today. The 4.5 was ripped at 2981ft so we set a 400ft plug. The Clinton was behind pipe but we still set 400 plug for good isolation at the bottom of the 7. The 4.5 will be pulled then the rig crew will go back in and rip the 7.
1438778258 2015-08-05 Plug / Plug Back No Violations I was on site to witness the plugging of the well. Today Artex had Formation cementing on location to set the 7 rip plug and the Berea plug. We set 100ft plug at the 7 rip (2098-1998) then pulled the tubing up to set the Berea. Berea plug was set from (1367-1217). The tubing was then pulled out of the well to let the cement set up. The rig crew will run tools down hole to tag the plug in the morning.
2092618484 2014-06-04 Preliminary Restoration Violations Noted The location has erosion issues and the operator needs to work on rebuilding the dike. There are two 210 bbl tanks in the dike. The well is currently shut in due to mechanical problems. I talked with Joe Schneider and he said he would have someone come out and till, seed, and mulch the location. He will also correct any dike and erosion issues. Site does not meet preliminary restoration requirements.
