0000211178 2018-08-29 Production Wells No Violations I was on location today to do a field inspection on this well. The well appears to be capable of production at this time. There is a pumping unit on this well with rods and 2 inch diameter tubing in it. The production valves coming off of the wellhead to the tank pad are open. The identification for the well is located on the production tank. There are no violations at this time.
0331791040 2013-06-18 Drill / Deepen / Reopen No Violations Artex has filled in the pit and leveled and graded the location. They will be coming back once ground dries out to remove production equipment and seed and mulch the area.
0766796045 2013-08-26 Drill / Deepen / Reopen No Violations The site still does not meet Preliminary restoration, however tractor was tilling the area at time of arrival. There was a trailer loaded with palletized lime, fertilizer and grass seed. There are three 100bbl tanks and a separator at the top of the access road. Identification is posted on the tanks.

They will be out either today or tomorrow to fill the pit. The wellhead is shut in until they install production lines.
1120094499 2014-05-27 Drill / Deepen / Reopen No Violations The well location still does not meet preliminary restoration status. The well is now equipped with a pumping unit which was active when I arrived on location. The access road and area around the well location is barren and operator must restore vegetative growth. I talked with Frank Riggle and he stated that they just lined up a contractor and will begin restoration work on all wells. He thought this well had passed restoration and will send someone over to reseed the barren areas.

1366509320 2013-04-20 Drill / Deepen / Reopen No Violations I was on location to witness the cementing of the 9 5/8 surface casing. The Killbarger IDECO Rig #17 drilled a 12 1/4 hole to a TD of 739. The casing shoe depth was 687. Centralizers were not run on the surface casing and the baffle insert was at a depth of 645. The operator encountered hard sandstone at 691-730. The plug was down at 11:57pm with good cement returns. There was a leak between the casing head and the landing joint collar and pressure was held for 10 minutes but it leaked off down to 400 psi. The cement remained at the surface. Jason, with Artex said they will do a 30 minute casing pressure test when they test the BOP tomorrow. 4/21/2013: Heath successfully tested the surface casing at 500 psi for 30 minutes then tested the annular for 15 minutes during the BOP test.
1368809211 2013-05-17 Drill / Deepen / Reopen Violations Noted Artex has finished treating the well. I went out to location to find a wellhead with a meter and chart. The trough around the wellhead had production contamination mixed with rain water. The pit was full of production waste and formation fluid and there was pit liner issues on the left side of the pit. I called Joe Schneider with Artex and told him of the issues and he said he would get a water truck out there right away and clean up the problem areas and take care of the pit issues.
1369845389 2013-05-29 Drill / Deepen / Reopen No Violations Compliance Action Follow-up 1368809211: Operator has filled in the trench leading from the wellhead to the pit and built up the edges of the pit. Most of the waste has been removed however there is still an area were there is a pool of brine. The brine is in the opposite side of pit from the breached liner. Quantab results show over 6000ppm of chlorides.
1953689252 2013-09-26 Drill / Deepen / Reopen No Violations Site does not yet meet preliminary restoration requirements. All drilling equipment has been removed and well is currently in production, however there are several barren areas which still need vegetative growth. I will continue to monitor this location.
