0380633670 2016-12-12 Production Wells No Violations The inspection today revealed that this well at one time had a pumping unit equipped to it and now the pumping unit is off the well and off to the side. The rods from when the pumping unit are still in the hole. The valves on the wellhead are closed at the time of inspection. The wellhead is equipped with a 4 1/2 inch diameter wellhead, 2 inch diameter steel production lines, and 1 1/2 inch diameter production tubing. There is a tank at the top of the hill from the well that is not associated with this well. The tank battery is equipped with a 100 barrel tank and a separator. The will is idle at the time of inspection and the issues have not been resolved. Pictures were taken at the time of inspection.
0549707695 2018-04-18 Production Wells No Violations I was on location today to do a follow-up inspection on the compliance notice that was issued for the well being idle and incapable of production. At this time there has been no resolution on getting the well back into production. The well is still sitting idle with all the production valves shut at the wellhead. The pumping unit is no longer connected to the well. The last time production was reported on this well was in 1999. The well is still in violation at this time. I called the company and left a message during my inspection on the well.
0662373865 2015-12-30 Production Wells No Violations On my inspection today I found this well to still have the pump jack removed from the well head. The polishing rod is still in the well head and all valves on the well head are closed in. There has been no resolution to the issues.

Mike has purchased this well and others from Luke Mallett. Larry is in the process of trying to get wells cleaned up and back into production.
0996229775 2015-12-01 Production Wells Violations Noted On my inspection today I found this well to be idle and not producing and the valves are all shut at the well head and the production tubing is disconnected. I spoke with Angus Energy on the phone and informed them that the well was idle and needed to be plugged within 180 days and a plan must be submitted within 30 days. They told me Mike would call me back and discuss with me the plan they would be taking. The well owner/operator shall submit a written plugging schedule to the Brian Sefciks office at 5880 Memory road, Zanesville OH, 43701 within 30 days and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. Brian Sefcik can be contacted via his cell phone at 740-255-2833. Also on my inspection the land owner talked to me about possibly wanting to buy the well off Angus to put back into production.
1230468254 2017-01-12 Production Wells Violations Noted The inspection this day revealed that this well is equipped with a 4 1/2 inch diameter wellhead and 1 1/2 inch diameter production tubing. The well currently has the rods sticking out of the wellhead and the pumping unit is off the well and sitting to the side of the well. The valves on the wellhead are closed at the time of inspection and there is an electric box near the wellhead that would be used for the pumping unit motor but it is not in operation at this time. I spoke with David Richman from Angus Energy about this well to see if he did know it was idle. David informed me they did indeed know it was idle and were unsure of what Angus was going to do with it as the landowner Claud Janes had expressed interest in buying the well about a year ago. At the time of inspection there has been no resolution to the idle well compliance notice on this well. Updated pictures were taken at the time of inspection. Angus Energy LLC shall submit a written plugging schedule within 30 days to Brian Sefciks office at 5880 Memory Rd Zanesville Ohio, 43701-9553 and plug the well within 90 days. Angus Energy LLC will be notified if the plugging schedule is approved or denied.
1379347770 2013-09-16 Production Wells Violations Noted Upon inspection, I found the well shut in and the pumpjack completely overgrown. The tank battery and separator were up on top of the hill and completely overgrown. The meter box was disconnected and the tank had no identification. There has been no production reported since 2009. Production has been reported as zero from 2000-2009. Operator must post proper id and turn in missing production to the state. Operator must make well capable of production within 60 days; or plugging is required in 60 days. Luke Mallet called and stated he already sold the wells to someone in WVA and has been waiting for the new owner to get everything in order with the state. I am going to wait and see how things unfold with new owner.
1416334956 2014-11-18 Production Wells No Violations Compliance Action Follow-up 1379347770: Pumpjack has been cleared of vegetation but the well is still shut in and the tank battery is still overgrown with vegetation. There is a separator and 100 bbl tank within the dike. There is no meter in the meter box. There has been no resolution to any of the issues.
1484322012 2017-01-12 Production Wells No Violations This Compliance Notice has been resolved, due to change of ownership. A new Compliance Notice will be issued for all outstanding Violations.
2006784585 2018-09-20 Production Wells No Violations I returned to this well today to do a follow-up inspection on it for being idle and incapable of production. At this time the well is still idle and nothing has been done around the well to get it back in production. I never did receive a phone call back on my last follow-up inspection. I contacted the company and spoke with the secretary today. She took all of my information and will pass it on so I can find out what the plans are with this well.

At this time the well is still idle and nothing has been done around the well to get it back in production. I called Angus Energy and talked with the office manager. They said they would have the field production call me to discuss the well.
