0134014622 2012-10-31 Production Wells Violations Noted This well was part of the complaint inspection for #21937. Upon inspection I found the well to be incapable of production. There was a pump jack at well head which was incapable of pumping due to disabled motor and broken belt. The pump jack is in poor condition. The well was equipped with 8 and 4 1/2 casing as well as tubing and rod. All of the plastic lines running from the wellhead to house and tank battery at the road have been cut and plowed up by landowner with a bulldozer within the last week to gain access to the wellhead and pumpjack. The valves were all turned on at the wellhead and the landowner stated there has been no production out of this well and the house gas was shut off in 2002. The Tank battery was freshly painted. There was no current chart on the meter and the steel sales line was disconnected and damaged. There are two 100 barrel tanks and one 450 gallon plastic drum in battery. There is oil contamination in dike which appears to be from plastic drum. There is no ID at wellhead or tank battery. Stonebridge is currently and has been reporting production on this well. Stonebridge needs to plug the well. Failure to comply may result in forfeiture of bond.

0799809702 2015-07-28 Production Wells No Violations I was at this well to do a follow-up inspection on it. At this time the well is not capable of production. The well is still in violation at this time. I took pictures off my findings during my inspection.

The production lines are still broken and scattered across the property all the way downhill to the wellhead. The valves are shut on the separator. There is no motor on the pumpjack. The landowner was present and stated that the yellow plastic lines were his and that his father had bought and laid those lines a long time ago for domestic gas. There is a 55 gallon drum adjacent to the wellhead which is starting to leak.

1221745410 2016-11-07 Plug / Plug Back No Violations I was on location on 11/7/2016 to witness the well plugged by Huffman & Bowers. Barry set a Clinton Sand plug (4630-4230 feet), a Big Lime/rip plug (3520-3320 feet), a bottom of surface plug (1740-1540 feet), and a surface plug (200-0 feet). Barry was able to rip the pipe off at around 3520 feet. They then pulled one joint to about 3479 feet where we set our rip plug. All 4 1/2 inch diameter casing was pulled from the hole after plugging was finished. The Berea was covered by the surface casing and no individual plug was needed.

I will refer this well to supervisor for legal review.
1478180136 2016-11-03 Plug / Plug Back No Violations I was on location to plug this well. Barry Bowers was the plug and cementing operator on this well. Barry had called me previously to say the rods and tubing were stuck in the hole. He then called me to inform me they had gotten them free and were going to plug Thursday November 3rd. When I arrived on site he was unable to get the 1 1/2 inch diameter tubing back to the bottom of the hole. I then contacted Inspector Smith to see what should be done. He suggested running the tools to the bottom of the hole to unblock whatever it was. Barry was not comfortable doing that in fear the tools would get stuck. I then called Supervisor Conrad and he suggested milling the hole. Barry wanted to try to work the tubbing to the bottom of the hole before trying that. No other updates to report at this time.
