0047615301 2013-03-25 Production Wells Violations Noted Well inspection revealed dike filled with production contamination and water, and in some places there was sawdust. The tanks appeared to have recently spilled oil over into the dike. There was also oil puddle around the well head from a leaking stuffing box. There is a pumpjack, a separator, and two 100 bbl tanks. The dike is in poor condition. There is no identification at well location. I called and left a message with Dennis Ruffs wife and asked her to have Dennis call me to discuss the well issues. If well is producing commercially then annual production needs to be reported to the state and owner needs to switch well from domestic well status to producing status. Operator needs to cleanup all of the contamination and repair/replace the stuffing box. A proper ID needs to be posted at location. 3/26/2013: Denny Ruff called back and said he would clean up the contamination around the wellhead. He said the oil in the dike was due to the separator falling over and taking out the knockout tank. He cannot get in to clean out the dike until the weather dries out the area but he will resolve the issue. He also stated Ohio Oil Gathering buys the oil and water off the well but the gas is for his home.
0248954911 2018-10-16 Production Wells Violations Noted I was on location today to do a field inspection on this well. During the time of my inspection I was unable to locate any identification for this well. The well is required to have the identification posted either at the well or at the production tank. If there is more than one well producing into the same production tank the identification is required to be at both the well and the production tank. The identification must have the following on it. The owner, lease name, well number, permit number, county and emergency contact number.
0330797120 2019-02-28 Production Wells No Violations I was on location today to do a follow-up inspection for the identification on this well. The identification has been posted at the tank pad. The well is no longer in violation.
0352943451 2013-07-03 Production Wells No Violations Denny has removed contamination from the wellhead and dike area and moved it to the agreed upon location. He has surrounded it with a small dike and covered it with lime. Identification is posted at wellhead. It appears he has fixed the leaking stuffing box. Everything in the field in now in compliance, however he will still need to resolve issues with Columbus in regards to bonding and insurance.
0523608382 2015-05-20 Production Wells Violations Noted I was at this well to do a field inspection on it. During my inspection I found the water tank inside the dike is leaking from the 2 valve. The valve will need to be replaced or fixed by 6/22/2015 and the contaminated soil will have to be cleaned up and removed from the dike.

1280453029 2015-12-29 Production Wells No Violations On my inspection today I found this well To have the soil replaces and no signs of oil or brine within the dike. A field test was ran and showed low amounts of Chlorides in the water in the dike leaving me to believe it was rain water from recent storms. NOV had been passed and no other violations to report at this time.
1366728956 2013-04-23 Production Wells No Violations Compliance Action Follow-Up -47615301: Dennis Ruff has not taken any action to clean up contamination or resolve bonding and insurance issues with the state. I called and left a message explaining the issues of contamination, bonding, and insurance.
1368801179 2013-05-17 Production Wells No Violations I met Denny Ruff today and we went over the issues at the well location. He is going to have someone come out and look at the wellhead to determine if there is a leak somewhere on the casing or casing collar. He said he cleaned up the oil contamination yet oil is still present. He did seed rake and shovel scald areas and throw down seed and straw. He will clean up all contamination in the dike and replace with fill dirt/gravel. We found a location on his property where he will attemp to bioremediate the contamination inside of a dike. He will also rebuild the dike at the tank battery. I gave him the Form 9 and explained that he needs to be bonded and insured since he does not own the property at the well location. He will fill out paperwork and has already contacted his bank. He will also call Vickie McKinney in Columbus to straighten things out with the state.
1370266131 2013-06-03 Administrative Inspection No Violations I called Denny Ruff this morning to see how the cleanup was coming along and he stated they had started on addressing the issues. He had stated he had not fixed the stuffing box or removed the oil contamination around the wellhead. He still has not submitted the proper paperwork to the state.
1787187974 2019-03-15 Production Wells No Violations I was on location today to do a follow-up inspection on the identification for this well. The identification has been posted on the production tank. The well is now in compliance.
1973025357 2013-05-07 Production Wells No Violations Compliance Action Follow-Up -47615301: Denny Ruff has thrown seed and straw on two areas. He did not remove the contamination from around the wellhead or from within the dike. He covered the of the puddle of oil around the wellhead with straw. He still has exempt ownership issues due to the fact that the well is not on his property. There has been no resolution to any of the issues at this time. I went to his house and called and left a message asking to set up a time to meet at well location. I still have yet to receive a response.
