0540131200 2011-09-22 Production Wells No Violations Well still a mess around tanks and well - no change. Have received plugging application and Knox Energy says they will be on location in a couple weeks to plug.
0555141488 2013-05-20 Production Wells Violations Noted Upon inspection I found a small brine leak in the 1 line at the wellhead. The dike has two tanks and one has a leaking valve. There is also evidence of an overflow. There is contamination in the dike and around the tank. I called Ron Combs and he said he accidentally spilled brine when he was changing the line from one tank to another. He will cleanup the contamination and rebuild the dike. He will also repair any of the leaks in the line connections and the tank. 6/7/2013:Ron Combs called and said the brine hauler had not completely tightened the plug on the tank. He corrected the issue and will clean up up the area in the dike.

That explains brine kill area - last inspection found no fluid in the tank and no drip. Possible that tank is leaking or may just be the valve. Musted rusted line in dike area, no grass growing between tanks and well indicating to me that leak is not new. Let message that tank may need replaced, valve definitely needs replace and need Gator up around the well to remove/degrade the oil on ground. Well is producing, IDs are good at the tank. Also called and left message for pumper that I could not approve. Will send to Columbus to change status to PR.

1986542624 2012-11-20 Production Wells No Violations Well is capable of producing. There is a pump jack with an electric motor. The well is equipped with 8 and 3 casing as well as 2 tubing and rod. Rod is not polished and the casing has small leak. Small 1 plug is leaking oil. There are two 100 barrel tanks. Dike is in fair to poor condition. I called Ron Combs and let him know of the leaks and he said he would look at it. He also said well only produces gas but he pumps it about 2-3 times a year to get the water out and into the tanks. He is going to post a brand new sign displaying the well ID. ID is posted at this time.
2024209498 2011-03-30 Production Wells Violations Noted My inspection this day found the well equipped with a pumping unit, valves open and in operation. The pumping unit is situated on a 3 high soilpile. Surrounding the pumping unit is a large brine kill area which has been covered with hay. This kill area, which is approximately 10 wide and 100 long, continues over to the tank battery and then into the backside of the tank battery dike. The landowner has stacked pallets, wood, brick and cement blocks over the kill area but it is still visable. A well identification (ID) sign is posted but there is no information on it. It appears that the well is being used for domestic purposes. Our database shows no production has been submitted for 2010. All impacted soil must be treated in place or excavated and dispoded of properly. Proper ID must be posted at the well and tank battery. 1/30/12 - Issue has been RESOLVED - The landowner now owns well. Proper ID has been posted. Some site cleanup is still required.
