0809437249 2013-08-05 Plug / Plug Back No Violations I arrived on site 0900 hours. Appalachian well service on site 1200 hours. The ran a collar buster to 1685. No pipe could be retrieved. Formation set a berea/rip plug of 200 at 1685. Appalachian well service shot off the 4.5 casing where Knox Energy found free pipe. No pipe was able to be retrieved. Formation set a 200 rip plug at 1158. Appalachian well service shot off the 4.5 casing at 800. The well head parted from the surface casing. They will remove well head on 8/6/13 and proceed with the surface plug.
1201434500 2014-06-10 Plug / Plug Back No Violations This site did not meet final restoration requirements because the roadways were not restored. There are flow lines left behind leading to the top of the hill and a steel flow line on top of the hill. Also at the beginning of the road way there is a separator, flow lines, fence and an 800 gallon poly tank left behind.

He also said they are going to try and get well back into production however they cannot get the service rig to that location until middle of July. If they run into problems getting it back into production then they will plug the well.
1375364479 2013-08-01 Plug / Plug Back No Violations Obstruction encountered yesterday in tubing at 3878 feet. 20 Barrels of fresh water were pumped at 200 psi into the tubing in an attempt to move the obstruction today. After pressures of 500 psi were encountered the pump truck was shut down. Tools ran down hole and the obstruction was tagged at 3874. Free point was found and the tubing was shot off at 3804, one joint above the Oriskany perforations which were at 3840. Three pup joints of tubing totaling 16 feet were pulled. An 800 foot cement plug was placed from 3788-2988.
1376329978 2013-08-12 Plug / Plug Back No Violations Compliance action Follow-up -1480664722: This inspection is for a follow up on an oil spill previously reported by Inspector Kip Thompson. There were no signs of pollution and contamination in the water under or around the bridge. The water was in fact very clear with no residual sheen. Operator is in compliance.

1480664722 2013-08-06 Plug / Plug Back Violations Noted Knox Energy spilled aprox 2 gallons of oil in Little olive Green Creek. They were removing water from the creek when the pump quit. This caused the oil to backflow out of the hose into the creek. I called OEPA spill hotline, incident number 1308-58-1799. I also contacted Mike Sharren to inform them of the clean up methods. Landowner Tammy Silvus was concerned about her cows dying. I showed the landowner the extent of the spill. It stayed under the bridge due to slow moving water. Knox Energy used booms and diapers and a vacuum truck to remove the majority of the oil. I told the landowner they would be a sheen left that natural sunlight will remove.
1849474566 2013-07-31 Plug / Plug Back No Violations Witnessed plugging activities for this well. Company was attempting to shoot tubing off at a depth of 5170 ft. Logging tools sat down at 3879 ft. After much collaberation with Supervisor Dan Goins, company man Steve Reyher, Logger John Hudson and Berry Well Services rig crew, they decided to pump on the tubing and see if they could gain circulation and see where the holes in the tubing were if any.
1944067954 2013-04-26 Production Wells Violations Noted Upon inspection, well was found shut in and production has been reported as zero since 2009. The chart at the meter was dated March 28th, 2013, and is for both the Phillis #1 & 2. The Phillis #2 is reporting production. There is a 1400 gallon plastic tank and two separators at beginning of the access road. Operator must file annual production, and must make well capable of production within 30 days; or plugging is required in 30 days. Failure to comply may result in forfeiture of bond. 5/6/2013: I talked with Steve Ryer and he is going to try and get the well permitted for a tempoary inactive status. They want ot get the well back into production.
