0056518008 2013-10-02 Preliminary Restoration No Violations Inspection found the well producing off the 4.5 production casing and a tank battery erected on site with a single 100 barrel production tank present along with a mechanical separator and gas sales meter with a current sales chart in the meter. I also found the restoration unsatisfactory due to the fact that proper soil was not brought back onto the disturbed area that will support vegetation, only the compacted red clay is present. Preliminary Restoration not passed.

0401623542 2013-07-03 Casing String Witnessed No Violations On this day I witnessed the running and cementing of 930 of 7 surface casing with Universal Well Services cementing the casing. During displacement the casing pressure was increasing and once we had 2 barrels of good cement returns to surface the decision was made to stop displacement at 450 psi and 30 barrels - 7 short of actual due to a increasing pressure. No wiper plug casing shoe or float collar was ran. Casing shall be pressure tested before drilling out from under the surface casing.

I spoke with Matt Longwell in regards to some of the other well sits that did not have good management practices initiated when the sites were built. Matt is going to get with Becky Sims and discuss this issue.
1372811307 2013-07-02 Casing String Witnessed No Violations The said well was spudded on this day and 100 of 9.625 conductor was set and cemented to surface with 10 barrels of cement returns to surface. The mix water was tested with no chlorides shown on a low level test strip.
1373572744 2013-07-11 Administrative Inspection No Violations On this day I picked the long string cement and casing records from Beck Energies in County office. Becky Sims and I reviewed the drilling completion records. The well is to perforated during the week of July 15th.
1551809708 2019-03-05 Preliminary Restoration No Violations Status check inspection found a substantial slip area below the well site. The area affected is between the well site and Township Road 6 Cochran Hill Road. The area affected is where the overburden was pushed during the well site construction. I photographed the area and contacted Beck Energys office and reported the slip to them, also a company field representative identified as Donnie Coe contacted me and the issue was discussed. Mr. Coe will work with the lead County Inspector Tom Thomas and address the slip. ODNR Engineers were also notified by Email of the issue and asked to provide support. The well is equipped with 4.5 casing with a reducing swage on the casing selling into a 2 sales line. The well is shut in. Also present on location is the production facility consisting of a single 100 barrel steel tank along with a mechanical separator and gas sales meter.

1585321333 2018-08-16 Preliminary Restoration No Violations On this day, I am conducting a preliminary restoration inspection and found the site meets ORC 1509.072 (A) with vegetation established and no substantial erosion. The well is equipped with 4.5 inch diameter casing and a 4x2 inch diameter swage with the production valve open. It is produced as an open-flow well. The production facility consists of (1) 100 barrel steel tank, drip tank, vertical separator, and an orifice sales meter with a current chart. The production records are current, but the identification needs updated. The company was recently contacted about identification, and the issue is currently being resolved.
