0258207896 2010-06-20 Drill / Deepen / Reopen No Violations A Michelle Debolt called a complaint on 6-19-2010 in on a large amount of slur going down a creek near where her livestock is kept. Address is at 41600 Trail Run rd.(cty rd 12 in Benton twp.)She states that she has to pack water to animals. Call was sent at 9:30pm. She states that as dry as it has been that a large amount has came down creek with a red tint. There is a heavy sediment. She had called the Monroe Sheriff Department and a deputy Rinard was sent and then OEPA hotline was called.Stonerock could not reach me and sent Dave Ball to inspect the situation. I called after getting message.

Dave Shot called and wanted to know what would be required of the State Of Ohio for vegetation. Harry Cisler called him and stated that they only had till Oct.15 to get location established w/vegetation. They would do a no till approach.
0773170124 2018-04-04 Production Wells No Violations On this day, I found the well equipped with 4.5 inch diameter casing with the valve open and 2.375 inch diameter tubing with the valve open. It is being produced by means of a pumping unit with an electric motor, tubing, and rods. There are (2) 210 barrel steel tanks, a drip tank, horizontal separator, and an orifice meter all contained within an earthen dike. The tank is properly identified and the production records are current.

1278508985 2010-06-21 Completion Testing Violations Noted On day of inspection the Marcellus frac pit was cut with a track hoe. Rob Armstrong was on the track hoe and son Mike Armstrong was on the dozer. They were told to close the gash over and stop releasing the water. The large pit that was containing over 2.5million gallons of water was 2/3 of the way empty. Samples and pictures were taken. There were four 500bbl tanks and two were being used for flowback.Two trucks that had Alpha drilling on the door removed two of the four tanks.

1397105993 2010-06-21 Drill / Deepen / Reopen No Violations On this day of inspection of the said complaint I had stopped by the sheriff department and talked to them about the call that was called in by Michelle Debolt.Dennis Deavers called and also let me know. I went to complaintants house to meet with her and get her story. Then I went to Harry Cisler no.3 to check the pit where there was going be a large Marcellus frac and found that Rob Armstrong cut a large cut with a track-hoe about 10x15tall and let the pit go. The red sediment was from the pit wall that was cut and flushed down the creek. Pictures and samples were taken.
