0020979440 2002-07-31 Production Wells No Violations Status check inspection - Oil stained soil and gravel in and around the containment dike. The well and tank battery have no identification. The electric wire has become detached from the fourth utility pole from the well head. The electric needs to be reattached and the well head and tank battery need identification.
002155i284 2002-09-04 Production Wells No Violations NOV follow-up inspection indicated that the requested actions have not been completed. These actions included spill remediation in the tank battery, wellhead and tank battery ID and repairing a downed electric line. I contacted Greg Miller with Virmurco while at the site. He stated that they have been waiting for Resource America to complete the required work and that Resource should be on site the week of September 9.

0817853629 2014-05-01 Production Wells Violations Noted Inspected production well on 05/01/2014. The well is equipped with a pump jack with an electric motor located within an earthen dike. The diked area is located within an unlocked metal fence. The well identification had fallen off the well and was laying on the ground. No tanks were noted nearby. Research into the production of the well showed that the well has not reported production to ODNR since 2010 and that the well has not produced since 2010, therefore the well must be plugged.
0883735107 2011-12-02 Production Wells Violations Noted The Uniontown office received a call from Kurt Weaver (330-723-2551) at 6979 Wooster Pike in Montville Township reporting dead vegetation adjacent to a tank battery on his property. The inspection found no identification on the tank battery or the wellhead and a brine scald that extended from the tank battery to the east and northeast approximately 60. High range Quantabs reached their maximum response over 6300 PPM chlorides. While conducting the inspection, Greg Miller with Virmurco arrived on site. We discussed the corrective actions that are required and the timeline to complete. Identification will be posted at the well and tank battery within 24 hours. A vac truck (S&H) will recover standing water in the impacted area and the surface will be scraped with the soil disposed at Petrocell. Seed and mulch will be applied as soon as weather permits.
1037939432 2012-01-09 Production Wells No Violations The chloride level in the water adjacent to the tank battery is reduced from Quantab saturation to 3340 PPM. The impacted soil has been removed. The site will be checked in the spring to verify continued dilution of the release and any lingering affect on the vegetation.
