0377138316 2014-06-17 Production Wells Violations Noted Inspected production well on 06/17/2014 with Brittany Hupp. The tank battery consists of 1-50 bbl steel tank, 1-1,600 gal plastic tank, drip, and 1-vertical separator within an earthen dike. The diked area is located within a wooden privacy fence. A small fluid leak was noted at the piping connecting the two tanks that will need to be addressed. The tank battery is used by multiple wells; identification is posted for all wells utilizing it, however no permit numbers are listed. The well is equipped with a pump jack with an electric motor located within a wooden privacy fence. Well identification is posted at the wellhead as required. Research into the production history of the well showed that no production has been filed since 1993, therefore the well must be plugged.
1198075609 2007-12-19 Production Wells No Violations Well is in production and equipped with pump jack, rods and tubing. Well is used to heat the health club on property. Tank battery has a 50 barrel steel tank and a 35 barrel plastic tank with a separator. Cloudy, 35 degrees.
1348771319 2012-10-01 Production Wells No Violations I inspected this well and found one pump jack unit with electric motor at the well. I could not determine the pressure on the casing or tubing as there were no gauges installed. A wooden privacy type fence was around the pump jack and wellhead and there was an excessive amount of weed growth within the fenced in area. The identification sign on the well had the wrong lease name, wrong well number and no owner name on it. The sign currently identifies the well as the WCAC #2 when it should be identified as the Tru-Fit #6A. I also inspected the tank battery and noticed that there was no identification sign on it either. The tank battery contained one 50 bbl. steel oil storage tank, one 50 bbl. plastic brine tank and one horizontal oil and gas seperator. The dike surrounding the tank battery was in good shape and there were no leaks detected around the tanks or piping. There was an excessive amount of weed growth within the tank battery. I met with Jim Steger, the maintenance manager of the Washington Court Athletic Club where these wells are located and discussed my inspection with him. He advised me that he will get a new sign made with the proper identification markings for the well and tank battery and do some weed control. The new signage should be installed within ten days. I also discovered that the production reports for this well have not been submitted since the 1990s. I contacted the well owner, Bob Workman, and advised him that he will need to get these past production reports submitted to the ODNR to bring the records up to date. Bob Workman advised me that he wil get this done as soon as possible.

Will contact Mr. Workman with Tru-Fit to determine his plan forward. No other problems were noted at the time of the inspection.
