Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
000PL11678 Preliminary Restoration No Violations Preliminary Restoration done information from Data Point
1312894720 2014-10-27 Production Wells No Violations Inspected production well on 10/27/2014. The tank battery consists of 1-210 bbl steel tank, 1-100 bbl steel tank, 1-50 bbl steel tank, 1-vertical separator, drip, and meter assembly located within an earthen dike. The well is equipped with a pump jack with an electric motor. Well identification is clearly displayed at both the tank battery and the wellhead. No problems were noted at the time of the inspection.
1366150770 2013-04-16 Production Wells No Violations I inspected this well and found one electric powered pump jack on the wellhead. I could not determine the pressure on the tubing or on the casing as there were no gauges installed on either. There was a small well identification sign mounted on the pump jack, but it incorrectly identified the well owner as Lomak Petroleum when the current well owner is actually D.A. Waldron & Assoc. There was a residential type gas meter with a regulator that was plumbed into the casing and was supplying gas to a residence that was located some distance away from the well. No leaks were seen on the wellhead or on the adjacent piping around the wellhead. There was an earthen dike constructed around the wellhead and pump jack and the area around the well was nicely maintained. The tank battery contained one 50 bbl. steel oil storage tank and one horizontal oil and gas separator unit. There was no identification sign posted on the storage tank or anywhere else in the tank battery, but there was a proper identification sign posted for this well at the entrance of the lease road. The dike was in good shape, but the area within and around the dike contained an excessive amount of high weed growth. I contacted Jerry Reeves of D.A. Waldron & Assoc. and advised him of the incorrect well identification sign on the pump jack.
