0445367327 2014-06-17 Production Wells Violations Noted Inspected production well on 06/17/2014 with Brittany Hupp. The tank battery consists of 1-50 bbl steel tank, 1-1,600 gal plastic tank, drip, and 1-vertical separator within an earthen dike. The diked area is located within a wooden privacy fence. A small fluid leak was noted at the piping connecting the two tanks that will need to be addressed. The tank battery is used by multiple wells; identification is posted for all wells utilizing it, however no permit numbers are listed. The well is equipped with a pump jack with an electric motor located within a wooden privacy fence. Well identification is posted at the wellhead as required. Research into the production history of the well showed that no production has been filed since 1993, therefore the well must be plugged.

1190640249 2012-10-09 Production Wells No Violations I inspected this well and found one small pump jack with electric motor on the wellhead. I could not determine the tubing or casing pressures due to no gauges being installed on the well. No visible signs of leakage were detected at the wellhead or on adjacent piping. The pumping unit was enclosed with wooden privacy fencing and there was an excessive amount of weed growth within the fenced area. A sign on the well incorrectly identified it as the WCAC #1 when it should have been identified as the Tru-Fit #1. The tank battery contained one 50 bbl. steel oil storage tank, one 50 bbl. steel brine tank and one oil and gas seperator. The dike was in good shape, but there was an excessive amount of weed growth within the tank battery. No visible leakage of tanks or piping was detected. I did notice that there was not an identification sign on the tanks or fencing. It was determined that this tank battery also handles the production from the Tru-Fit #2, the Tru-Fit #5 and the Tru-Fit #6A wells. I spoke with Jim Steger, the maintenance manager of the Washington Court Athletic Club where this well is located and advised him of the incorrect signage on the well and the lack of an identification sign at the tank battery. He advised me that he will have the proper signage made and installed within the next ten days. He also said that the gas produced from this well is being used in the buildings of the athletic club. No gas is currently being sold. I also contacted the well owner, Bob Workman, at Tru-Fit Products and told him that no production reports have been submitted for this well to ODNR since the 1990s. Mr. Workman said he will get the proper reports filed as soon as possible so that the records are up to date.
