0519109279 2014-03-04 Plug / Plug Back No Violations Visited location on 03/04/2014 to set bottom plug. Set bottom plug from 3,688-3,288 through tubing. Rig debated on setting additional plugs, so they decided to rip pipe at 2,441. Ripping took longer than expected, so the rig decided to to pull up to the bottom of the Big Lime plug, and shut down for the day. Plugging operations will resume tomorrow.
1102649356 2014-03-05 Plug / Plug Back No Violations Visited location on 03/05/2014 to finish plugging operations. Set Big Lime plug from 2,371-2,071, Berea plug from 713-437, and surface plug from 150-surface through tubing. All cement plug jobs went well and good cement was brought to surface. Plan forward is to reclaim location, cut well off 3 below ground level, weld on a steel plate, then cover with soil.
1382700119 2013-10-24 Production Wells Violations Noted Inspected well per the request of the landowner, Hazel Zegarac on 10/24/2013. Landowners son, Pete Zegarac operates the businesses on the property the well is located on. He indicated that the well hasnt been used in 20+ years and said that he had previously received notices from the state to plug or produce the well, but was unable to find anyone to do the work. I put him in contact with the Ohio Oil and Gas Association to find a contractor. The well location is ~0.10 mile behind the family businesses located on SR-18, east of Medina, and is very overgrown. The wellhead is equipped with a swedge and pressure gauge, as well as a 25 bbl steel tank. There was no pressure on the gauge. The tank is rusty, and the bottom of the tank is minorly corroded (no visible leakage). No well identification is posted at the well. The well is capable of production, but is clearly not being produced. Well must be produced or plugged.
