1287293455 2014-11-10 Final Restoration No Violations Visited location on 11/10/2014 to sign off on the final restoration for the Moss #2 well. The well has been plugged. All surface equipment at the well location has been removed, and the area has been rough graded. Production equipment (tanks, separators, etc.) remain on the lease, as they are being used by the Moss #1 well, which is still in production. Final restoration approved.
1351109113 2012-10-24 Production Wells No Violations I inspected this well and found one pump jack with electric motor on the wellhead. The pump jack looked as though it has not been in operation in quite a while. There were high weeds growing all over the pump jack and electric motor. I could not determine the pressures on the casing or tubing as there were no gauges installed on either. I detected no visible signs of leakage from the wellhead or piping, but there was an excessive amount of high weed growth around the wellhead. The tank battery was enclosed with a chain link fence and gate and contained two 210 bbl. steel oil storage tanks and one horizontal oil and gas seperator. The gas recording meter had a chart on it that was last changed on 10/18/12 and was recording flowage. The dike was in good shape and the proper identification signage was posted on the tank battery fence. The paint condition of the tanks and the seperator was poor and the tanks were very rusty and both tanks were empty. No visible signs of leakage was detected on the tanks or piping. It was determined that this tank battery also handles the production of the Moss #1 well.
1617154798 2013-12-05 Production Wells Violations Noted Inspected well on 12/05/2013 per the request of William Snow and Regran Leedy, who are realtors selling the property the well is located on. The tank battery consists of 2-210 bbl tanks, 1-100 bbl tank, 1 separator, and 1 production unit on a gravel pad within an earthen dike. The tank battery pad is located within a locked metal fence. Production equipment and tanks are rusty, but the tank battery pad and the lease road are fairly well maintained. Tank battery is used by multiple wells; well identification is posted at the tank battery for all wells utilizing it. Well is equipped with a pump jack with an electric motor on metal stilts. Well identification is not posted at the wellhead as required. Well was shut-in at the time of the inspection. A periodic strong odor of gas was detected, as well as gurgling coming from the wellhead. I called Bill Lavin with G & O Resources regarding this issue on 12/05/2013 and left a voicemail. We spoke on 12/09/2013; he acknowledged the well was leaking, but indicated he was unsure of where the leak was coming from. He stated that he was working on getting a service rig in to determine where the leak was coming from and repair it if possible. If it is found that the casing is leaking, he stated that the well would be plugged and abandoned.
1752752322 2014-02-25 Plug / Plug Back No Violations Visited rig on 02/25/2014 to witness the plugging of the Moss #2 well. 4 1/2 production casing was found to be parted at 2,548, and it was pulled from that point. The Big Lime plug was set from 3,628-3,228, the Berea plug from 640-435, and the surface plug from 150-surface. Cement plug jobs went well and good cement came to surface. Plan forward is to cut off the casing 3 below ground level, weld on a steel plate, and cover with soil.
