0002155i34 2002-05-02 Production Wells Violations Noted CASE # 02-021 Chris Holmes with the Ohio EPA received a complaint from Diana Pomeroy (5741 State Route 301, Spencer, Ohio) on April 19, 2002 concerning an oil and gas holding tank leak. The complaint was forwarded to MRM Uniontown via fax on May 1, 2002. The Pomeroys were contacted by phone (330-648-2193) on May 1 and a description of the problem indicated that natural gas was bubbling to the surface adjacent to the northwest corner of the Pomeroy property. The site was visited on May 2, 2002. Natural gas was found bubbling through a small puddle of water on the east side of SR 301, north of the northwest corner of the Pomeroy property and adjacent to a small horizontal tank (approx. 200 gallon size). Crude oil was not present on the water or the adjacent soils. The nearest producing wells are operated by S&S Energy (Roger Scott 330-472-8287). A natural gas pipeline, which according to a pipeline marker crosses under SR 301at the leak site may be the source of the gas. Neither the pipeline nor the crossing marker were identified. On May 2, Roger Scott was notified by phone of the potential problem with the request that the leak be repaired as soon as possible. Scott indicated that he would investigate and report back on Friday May 3. S&S determined that the source of the leak was the Brown Unit #1 well. Repairs were initiated on May 6 and will be completed as soon as possible. It should be noted that the Pomeroy property has an unidentified and unmapped Berea well, which is in the I&O program.

The well is equipped with a pump jack with an electric motor; the wellhead is enclosed in a wooden box. The electric line to the well is on the ground in several spots along the lease road. 2014 production showed that the well did not produce any oil or gas in 2014. Jason with S & S plans to have lines repaired/replaced in Spencer over the summer. 6-month extension granted to get lines repaired and wells turned back on.

The bottom of the tank furthest from the road is beginning to corrode and will need repaired. Well identification is clearly displayed at the tank battery. The well is equipped with a pump jack with an electric motor. The electric line was on the ground in multiple places and does not appear to be functioning. Research into the production history of the well showed that it has not produced since 2002, therefore the well must be plugged.
