0002155i79 2002-05-23 Production Wells No Violations CASE 02-024 The investigation began as a gas odor complaint forwarded to Insp Lowder by the OEPA (Ron Bonner). The inspection indicated that the gas source was the Brunswick Twp Trustees #1 (P-1337) tank battery which is used by the Anderson #1 well (P-1347). The gas is vented through one of the stock tanks. Conversations with John Williams discovered that the Trustee well is being used as an injection well in an unregulated and unpermitted enhanced recovery project. The injected natural gas is obtained from other Williams wells in this field which no longer have access to a conventional gas market. The compressor system, which formerly was used to sell gas is now used to inject gas into the Trustee well. The well is equipped with a pump jack, tubing and rods. Injection pressures exceed 300 psi (max pressure on the injection gauge). Conversations with Tom Tomastik (UIC) indicate that this enhanced recovery project is not permitted with UIC.

0009579349 2002-05-30 Production Wells No Violations Conducted research into injection well violation. Searched RBDMS for wells that are still in production and wells that have been plugged and abandoned. The original field was drilled by Natol Corp. (later Oxford Oil) in the early 1960s. The field was purchased by Williams in February 1972. At least 15 wells in this old field are either still in production or are not plugged and abandoned. RBDMS indicates that 14 wells have been plugged. Will initiate the investigation with Investigator Moravy the week of June 3, 2002.
002155i100 2002-06-05 Production Wells No Violations Meeting with Insp Moravy and John Williams re terminating gas injection into the Trustee well. We witnessed Williams shutting down the compressor and disconnecting the injection line at the wellhead. We explained the requirements for an enhanced recovery injection program and suggested that Williams contact Tom Tomastik at UIC if he wants to pursue this course. Other options that were discussed included obtaining the missing right-of-ways to market the gas or flaring the gas if a market is unavailable. We will continue to monitor the site for compliance.
0564367305 2016-07-07 Production Wells No Violations On this Compliance Issue Follow up Inspection I checked on the progress of the removal of contaminated soil and gravel from lease road up to the the tank battery, then the company plans to address the tank battery material

10:00 am I arrived the cleanup had started at approximately 200 feet downslope of the tank battery along the lease road uphill toward the tank battery. The contaminated soil and gravel was removed and hauled off site to haul Petro Environmental (slips to be filed). New booms placed in drainage ditch along the lease road. As the cleanup was started at the tank battery it was determined that oil storage tank bottom had been corroded and developed leaks and TENORM was a concern for material within the tank battery. Mr. Swain was advised that the material from the diked area of the tank battery would have to remain on site till it can be tested For TENORM. Mr. Swain indicated that he did not want to leave the material on site due to the forecast of rain and he placed a call and left message for Deputy Chief Bob Worstall. At approximately 12:00 pm Deputy Chief Bob Worstall returned call and granted Landmark IV permission to remove the material from affected tank battery area and be stock pile and contained at a landmark 4 facility

1212400652 2014-05-23 Production Wells Violations Noted Inspected production well on 05/23/2014. The tank battery consists of 1-100 bbl steel tank located within a shallow earthen dike. The diked area is located within a locked metal fence. A small amount of oil was noted near the valves at the bottom of the tank and will need to be cleaned up. A vertical separator is located nearby, outside of the dike. The well is equipped with a pump jack with an electric motor located within an unlocked metal fence. Well identification is clearly displayed at both the tank battery and the wellhead. Research into the production history of the well showed that no production has been filed since 2011, therefore the well must be plugged.
1473774903 2016-09-12 Production Wells No Violations On this Compliance Issue Follow up Inspection I check if remedial action have been made to the site, the are of the original tank battery has been filled in and reseeded Landmark IV crew has been working on the domestic line for the Metro Park buildings. An area just west of the original tank battery was selected for a new and smaller tank battery was constructed and the necessary lines the well have been plumbed into the oil storage tank which is within and diked area.

03:00 pm Received phone call from Steve Swain of Landmark 4 LLC, to report an oil tank that

 overflowed into dike and the pumper is on his way to site.

03:12 pm Voice mail from Terry Whitehair Emergency Response: to call him. 03:20 pm is working on getting an ER Coordinator routed to site. 03:42 pm Vikki Bunting Emergency Response Coordinator is in Mount Vernon and will be leaving soon to go to site. 04:00 pm I arrived on site and met Fire Department (Assistance Greg Smith) and their HAZMAT Team from the City of Valley View. The Brunswick Fire Chief (Anthony Strazzo). They had contained the spill by placing oil absorbent pads on the loose crude oil which had flowed over the tank battery dike and then downslope along the lease road. With the arrival of pumpers Rich Holshue and Jim of Landmark IV LLC with additional equipment and supplies they proceeded with additional cleanup and containment of the spill, when the vacuum truck from S&H Oils Field Service arrived they proceeded to vacuum up the pooled crude oil in the contained areas in interceptor trenches dug along the north side of tank battery and lease road. The estimated volume recovered 15 Barrels. Additional trenches were dug across the lease road at several spots and additional oil absorbent pads, and peat sorb was placed. Additional booms were placed in three spots along the drainage ditch runs which along the south side of lease road and into the wooded area. The site was contained and will check on till morning when additional cleanup will be continued.

Verbal Compliance Notice Issued For OAC 9-1-07 /ORC 22(A) due to the fact company Called me to report spill.

Received 2014 production data from Landmark (which was turned in to Steve Opritza). Production data shows production for both 2013 (158 bbl oil) and 2014 (196 bbl oil). As production has been filed and the well is currently in production, compliance notices can be closed.
