0016479454 2002-07-25 Production Wells No Violations Status check inspection. Records indicate that the last reported production from this well was 2000. The overgrown condition of the vegetation around the wellhead indicates that the pumping unit has not operated for at least that long. The well must either be placed back into production or properly plugged and abandoned.
0248120303 2012-11-08 Plug / Plug Back No Violations I was on location to inspect the cementing of the bottom plug. Petroset Cementing Co. arrived on location and set up their equipment. I had Petroset pump fresh water and break circulation through the 1.5 in. tubing. They then pumped 25 sacks of Class A cement with 2 % gel into the well. This bottom cement plug was placed between 3277 ft to 3064 ft. using 8 bbls. of fresh water for displacement. I weighed the cement with a cement scale and found the weight to be approx. 14.6 lbs. per gallon. Thompson Well Service then tripped the 1.5 in. tubing out of the well and rigged up a 5.5 in. ripping tool and began ripping the 5.5 in casing at 2500 ft. No telegraph sound was heard on the casing so the ripping tool was moved upwards. A good telegraph sound was finally obtained at 1850 ft.
0339089162 2012-11-13 Plug / Plug Back No Violations I arrived on site and found that the plugging operation for this well was completed. I was not able to be on site on 11/12/12 for the cementing due it being a State holiday. I met with the consultant for Landmark 4 LLC, Harold Moravy, and discussed the operations that had occured the previous day. Harold advised me that Thompson Well Service had pulled 1850 ft. of 5.5 in. casing and loaded it onto a trailer.The top 330 ft. of the 5.5 in. casing did have holes in it. The crew then ran the 1.5 in. tubing back into the well to a depth of 1850 ft. for the cementing of the Big Lime plug. The tubing then became plugged off with shale cavings so the 1.5 in. tubing had to be tripped back out and the two plugged joints were replaced. The 1.5 in. tubing was run back into the well to a depth of 1648 ft. because the hole had caved in to that point. I was advised of this situation the day before via telephone conversations with Harold Moravy. I gave permission to Harold to set the Big Lime plug at 1648 ft. due to the possibility of future hole cave ins. Harold advised me that Petroset broke circulation through the 1.5 in. tubing and pumped 80 sks. of Class A cement with 2 % gel and set the plug from 1648 ft. to 1348 ft. The plug was displaced with 3.5 bbls of fresh water. The service rig crew then pulled the 1.5 in. tubing to a depth of 550 ft. Petroset then began to pump again and broke circulation and pumped 55 sks. of Class A cement with 2 % gel and placed the Berea plug from 550 ft. to 350 ft. This plug was displaced with 1.5 bbls. of fresh water through the 1.5 in. tubing. The service rig crew then pulled the 1.5 in. tubing to a depth of 300 ft. Petroset then began to pump again and broke circulation and pumped 105 sks. of Class A cement with 2 % gel from 300 ft. to the surface. Clear water was returning before cement came to the surface. 1320350975 2011-10-26 Production Wells No Violations I received a gas odor complaint referral from Kathleen Scheutzow, Township Trustee (330-225-5389). The original complaintant is a homeowner on Plum Creek. Landmark4 is the owner/operator and continues to vent natural gas through the stock tanks on the days that they pump the wells, which is currently Monday, Wednesday, and Friday. The remaining days, natural gas is vented through a flare system or the wells are all shut in. The complaint was received on Friday, October 21, 2011 which was a pumping day. The wells were pumping during this site inspection on Wednesday October 26 and a small amount of natural gas was venting through the stack. On October 26 however, the wind was blowing from west to east and away from Plum Creek. I contacted Landmark and discussed odor control options. Charcoal canisters may be an option in this case.
1352828226 2012-11-05 Plug / Plug Back No Violations I inspected the plugging operation today and found that Thompson Well Service was on location to perform the plugging operation. Josh Thompson, owner of Thompson Well Service, was operating the service rig and managing the operation. The pump jack and associated production equipment had already been removed from the wellhead and the service rig crew was in the process of rigging up over the well. The crew was also in the process of excavating a pit to capture any fluids from the well and there was a 300 bbl. skid mounted water tank on site.
1352832314 2012-11-06 Plug / Plug Back No Violations I arrived on site today and found that Thompson Well Service was in the process of pulling the production rods from the well. A total of 3277 ft. of production rods were removed and loaded onto a trailer. The crew then began to remove the 1.5 in. tubing from the well. A total of 99 sections of 1.5 in tubing were removed from the well with each section being approx. 32 ft. in length. A total length of 3277 ft. of 1.5 in. tubing was removed. The crew then went back into the well to release the 2.875 in. packer that was set at 2989 ft. The crew then began to pull the 2.875 in. tubing from the well. A total of 500 ft. of 2.875 in. tubing was removed and crew shut down for the day.
1354122854 2012-11-07 Plug / Plug Back No Violations I arrived on site to inspect the status of the plugging operations and found that Thompson Well Service was in the process of constructing a 10 ft. x 8 ft. x 6 ft. circulation pit. After the pit was excavated, it was lined with a 12 mil. thick plastic liner. The crew then began to pull the remainder of the 2.875 in. tubing and remove the 2.875 in. latch packer and loaded it onto a trailer. A total of 96 sections of 2.875 in. tubing, which totaled approx. 3277 ft., was removed. Each joint was approx. 32 ft. in length. The crew then ran 3277 ft. of 1.5 in. tubing back into the well to the top of the Clinton formation. This string of 1.5 in. tubing will be used as a working string to place the cement.
1354126057 2012-11-09 Plug / Plug Back No Violations I arrived on site and found that Thompson Well Service was successful in ripping the 5.5 in. casing at a depth of 1850 ft. According to the on site consultant for Landmark 4 LLC, Harold Moravy, Petroset Cementing Co. was on site earlier in the day and set up to try and establish circulation for cementing the rip plug. Petroset circulated down the 5.5 in. casing and broke circulation at 0 psi. The cementing operation was stopped at that point and Petroset rigged down and left the site. Upon further discussion, Harold Moravy, Josh Thompson and myself determined that the 5.5 in. casing has holes in it at approx. 150 ft. The decision was made to leave casing on slips for the weekend.
1516975013 2017-11-30 Final Restoration No Violations Conducted a follow-up inspection with company personnel to check for possible contamination from the plugging operation, Steve Swain and Jim Schmidt. Met at the plugged well site. They showed me the approximate location of the well which was real close to the Latitude and Longitude (41.205049, -81.80570) on file in well data base of the Division of Oil and Gas Resources Management. I inspected a 30-foot radius from the given coordinates and found no evidence of Pollution and/or contamination or any evidence of any ruts left by the plugging operation. This area will be used as the staging area for equipment during the plugging operation of the four remaining wells on this lease. I concur with the passing of the final restoration for this well.

Well History: This well was plugged on 11-12-2012 was witnessed by Division of Oil and Gas Resources Management Field Inspector Brian Logue also a final restoration inspection was conducted by DOGRM Field Inspector Rick Kepler on 08-01-2013 and another FR inspection was performed on 08-02-2013 by DOGRM field Inspector Amada Trimble.

1628104815 2013-08-02 Final Restoration No Violations There was no indication of the well or tank battery. The grading and vegetation have been restored to natural conditions. There are other producing wells on the property. The id sign for permit# 21312 was still posted on the gate to the lease road, and the lease road is still in use for acess to the other wells on the property. Final restoration passes.

Landmark is laying new production lines from wells to tank battery. Domestic Oil is the operator and Landmark LLC is the owner of the wells. The lease has not been transferred from Quality Oil and Gas to Landmark LLC as of today. I advised Steve Swain this must be completed by October 15, 2009. He called his land agent today, to go to recorders office and complete the transfer.
2146233671 2012-11-07 Plug / Plug Back No Violations At the time of my inspection I was on site with Medina County Inspector to review the plugging procedures for the well. The plugging crew had removed all of the production tubing and then ran 1.5 tubing to 30 above the total depth in preparation of setting the bottom plug. The crew was unable to get cement that afternoon plugging operations were put on hold till the next morning.
