Well was plugged 4-11-1966 by The Ohio Fuel Gas Company which is Columbia Gas Transmission. Well is vaulted and vented outside foundation of home at 3275 State Road, Medina, Ohio. Home is for sale by owner, Robert Cole. No visable leakage of gas at vent. Called Veronica Campbell to advise of findings. Rain, 39 degrees. Well location is in residential area.
1181046259 2007-06-04 Orphan No Violations Called by Ray Campbell to inspect noise from vent attached to a plugged gas well. Three inch pipe is attached to top of well and runs above roof top of his home. Vent has a rain cap and is allowing gas vapors to escape. Noise is gas bubbling through fluid, trapped in vent pipe. No evidence of leakage around well in yard, twenty feet north of house foundation. Tested inside and outside of house foundation with gas detector and found no evidence of methane. Advised Ray Campbell to monitor vent. If well becomes more active call ODNR to apply for Idle and Orphan funding to have well replugged. It was plugged in 1966 by Ohio Fuel Gas Company. Plug report on file. Rain, 80 degrees.
1326976053 2012-01-18 Plug / Plug Back No Violations Requested by Tom Tugend to meet with current landowner, Ray Campbell and inspect well. Plugged gas well is located about 10 feet north of house and is buried and not visible at surface. No dead grass or burn areas are reported by landowner. Vent pipe is attached to north side of house and is constructed through the soffit with terminus above the roof line. Approximately four feet above grade, a connection joint has separated and is open about 1/8th of an inch, due to settling of the structure and yard. Gas readings were collected with a Sperian gas meter with an automated pump attached. The following readings were observed; oxygen normal at 21.1 % for minimum and maximum, carbon monoxide was zero, Hydrogen sulfide was zero, Lower explosive limit exceeded readings, Methane exceeded the readings. The domestic water well is located near the front of the house to the east and approximately 40 feet from the gas well. The GPS coordinates are N41.18692 W81.71026.
1327005171 2012-01-18 Administrative Inspection No Violations This inspection report is intended to clarify observations made for this well on the same date. The gas readings collected with the Sperian multi gas meter were collected from inside the vent pipe where the pipe has parted. Two sets of readings were collected. The first readings were collected with the probe in the vent pipe and pointing upward toward the roof. The maximum readings observed were; oxygen 21.1 percent for the minimum and maximum value, carbon monoxide was zero, hydrogen sulfide was zero, lower explosive limit was 7 percent, and methane was zero ppm. The second set of readings were collected with the probe in the vent and pointing down the vent tube toward the well. The maximum readings observed were; oxygen 21.1 percent for the minimum and maximum value, carbon monoxide was zero, hydrogen sulfide was zero, lower explosive limit exceeded the manufacturers readings (this represented 100 percent of the LEL), and methane exceeded the manufactures readings and is greater than 200 ppm CH4.
