0796225578 2013-08-12 Completion Testing No Violations I received a phone call from Doug Oaks that FESCO the company flow testing the above well ran a 500 barrel frac tank over and about 8-10 barrels of flow back / condensate got out of the contaminate and onto the ground. A water truck was on location and vacuumed up all fluids on the ground. A contractor is on location with a back hoe and rollbacks are in place to remove contaminated soil and properly dispose of. The area effected was 6 wide by 80 long. They will be testing the depth of the soil also and removing if needed.

1083905499 2013-04-25 Drill / Deepen / Reopen No Violations I was on location for the cementing of the 13 3/8 casing. The drill depth was 1046 ft, with a 17.5 inch hole and 13 3/8 inch conductor casing placed at a depth of 1036.22 ft. The proper circulation was established and cementing was started by Halliburton using Class A cement with 50 % excess brought for the job. The beginning mudscale of cement was a density of 15.6 pounds per gallon (ppg). The amount of returns were determined to be 10 BBLs. The well site was in good repair with the proper containment in place. The lease road and well pad were in stable condition. The permit was in the company mans trailer. Fire extinguishers were readily available during the job.
1144639876 2013-06-06 Completion Testing No Violations I was on location to observe the construction of the frac head and values and pressures testing on the above location. This is a pretest before fracing. The 13 3/8 surface and 9 5/8 intermediate were installed with pressure gages to monitor the pressures. The 5.5 production casing was loaded with fresh water and a pump was installed to perform the hydro test. The 11634 of P-10 5.5 casing was tested @ 12500 psi for 30 minutes test passed there was no psi on the 13 3/8 or 9 5/8. The three values on the frac head were tested to 15000 psi. and passed the test.

1365691114 2013-03-13 Drill / Deepen / Reopen No Violations North East Region Supervisor Tom Hill and myself performed a pre-site application permit inspection with Halcons personal, Mike McCadle Construction Supervisor and Gregg Stewart Regulatory Specialist. I GPS the well stake ( 40.112150 -80.897060 ) All wet land were identified and marked. All ID signs to be posted at the Bailey Court West entrance. I will be notified before site construction begins.
1368556997 2013-04-22 Drill / Deepen / Reopen No Violations Patterson Rig 256 drilled a 26 open hole to a total depth of 630 and ran 616 of 18.625 J-55 BTC 87.5# casing pipe with 4 centralizes. Pattersons Rig 256 ran back in the 18.625 casing with a string of 5 drill pipe and 3 joints of 2.875 tubing and latched onto a cement stinger in the bottom of the cement shoe. Halliburton pumped 1196 sacks of standard blend 15.6 cement / 1.22 yield cement and displaced with 5.5 barrels to clear drill pipe and 2.875 tubing of cement. Good circulation and 12 barrels of cement returns to surface.
