0259916423 2018-07-12 Production Wells No Violations The purpose of my inspection was to document the status of well head identification and contaminated material clean up within the dike of the tank battery. At the time of my follow-up inspection, correct identification was present at the well head. The old contaminated material was cleaned up and removed from the interior of the tank battery. No further action is necessary.
0412504543 2018-05-23 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-2503. ODNR-DOGRM Northeast District Supervisor Stephen Ochs was present during the inspection. Access to the well and tank battery remained drivable and stable. No identification was present at the well head. The 8.625 well head was below grade. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. No leaks were observed. Staining consistent with a legacy operation was present. A stable earthen dike that was free of fluid was present around the production equipment. Equipment within the dike consisted of one (1) horizontal separator, one (1) 100 bbl steel production tank and one (1) 1200 gallon poly brine tank. The separator remained in good repair and contained a current 16 day production chart. The production tank remained in good repair and contained correct identification. Valves were closed and plugged. The poly brine tank remained in good repair and was holding fluid. The ground between the tanks contained staining consistent with a legacy operation. The inlet valve was closed. Following the inspection I spoke with Enervest field superintendent Larry Lower concerning well head identification and staining within the tank battery. Mr. Lower stated that he would direct the lease operator to post identification as soon as possible and remove the old stained material. I will schedule a follow-up inspection to document the status of identification and clean up.
0660888632 2005-03-16 Production Wells No Violations The tank battery consists of 1-100 bbl storage tank, 1-1,300 gallon plastic salt water tank, 1-separator and a dike.
1262023033 2009-12-28 Production Wells No Violations I performed a random field inspection. I GPSd both the tank battery and the wellhead. I continued to walk around both the wellhead and the tank battery and found both to be in good shape. There were no violations at this time.
