0643803059 2008-09-18 Production Wells No Violations I performed a random field inspection. I walked around the well and the tank battery and GPSd both. The ID sign was posted. There were no violations at this time.
1449072979 2015-11-04 Production Wells No Violations On 11/03/2015 at 7:30 pm. I was notified by Josh Shrock from Enervest that they had a release of brine at the above location. He stated that they had a J Hawks water truck on location and recovered some of the brine and secured the site until day light. I arrived on location on 11/4/2015. and discovered that the 2 nipple between the 210 barrel tank and the 2 value has been leaking for an unknown time and it had leached out from under the dike. I instructed them that they would need to remove 210 barrel and the 50 barrel tank and remove all of the contaminated soil and properly dispose of all materials. Sutton Pump was on location to steam out tanks and remove from location. They will start removing contaminate material today. The area outside of the dike that was impacted is approximate 30 ft. wide at the widest point tapering off to 1 ft. by 80 ft. long and approximate 6 deep.
1449682602 2015-11-05 Production Wells No Violations I was on location with Northeast Emergency Coordinator Terry Whitehair coordinating the digging and removal of the contaminated material, all material will be taken to an approved landfill with a manifest prepared by Enervest Operating Company. Sutton Pump was on an location with an excavator loading out continameted material. There were a total of 3 truck loads taken to the approved land fill The soil was removed to the clay Quantabs were used to determine when to stop digging. All contaminated material out side of the dike was also removed. The landowner was also on location and was satisfied with the clean up. EnterVest will seed the area that was impacted. I will inspect the location on 11/9/2015
