0626227347 2011-03-30 Production Wells No Violations I met with the contractor that is going to clean up the mess at the tank battery area(Bruderly Excavating), Kurt Bruderly owner. They were starting to take the 400 barrel tanks out so they could steam clean the inside for hauling off. There were no violations at this time.

1126652902 2012-01-25 Production Wells No Violations I conducted a status check on this site and found that the well is equipped with a pumping unit and connects to a tank battery that consists of one 200 barrel stock tank, a concrete underground brine storage tank and a separator. The thief hatches were closed and there was a bull plug installed in the draw valve. The dike was free of vegetation and debris. Proper identification was posted. I found no leaks or spills at the well head or the tank battery. I took new GPS coordinates of the well head and tank battery. No violations at this time.
1301595582 2011-03-31 Production Wells No Violations I performed a follow-up status check on the static electricity blow up on the 2 400 barrel stock tanks. The stock tanks have been hauled off. The cleanup crew were draining any liquid residue from the bottom of the dike area into a 5 gallon bucket. This cleanup continues. There were no violations at this time.
1545985925 2011-03-29 Production Wells No Violations I received a call from Tom Basick(330-575-9248) about a stock tank that blew. Static electricity was the cause of this accident. I and inspector Chris Grimm investigated this incident. Both 400 barrel stock tanks bottom and top were swelled out. Approximately 25 gallons of crude oil blew out the 2 inch bottom line inside the dike. Tom had started the cleanup when we arrived on the site. The tanks will be hauled away tomorrow and the oil, rocks inside the dike will be hauled off. There were no violations at this time.
1986691439 2008-10-27 Production Wells No Violations I performed a random field inspection. The well and the tank battery were GPSd. I walked around both the well and the tank battery and found it to be in good shape. The ID sign was posted. I found no violations at this time.
