1313766230 2011-08-18 Production Wells No Violations Met with Allen Summers of Enervest Operating at 08:00 on 08/18/2011 at leak site. All contaminated soil and vegetation removed and stockpiled for disposal, dike/berm constructed around leak site, double siphon dam constructed at recommended position along stream, absorbent booms and pads placed upstream and downstream of siphon dam. Water truck on site to vacuum oil skim from surface of water collected in natural pool upstream of siphon dam. No visible oil skim or sheen on water downstream of siphon dam. I performed pH and chloride test on water collected upstream of leak site to establish baseline water conditions. Results: pH - 6.0, chloride count - 40 ppm. Repeated pH and chloride test approximately 25 downstream of siphon dam. Results: pH - 6.0, chloride count - 40 ppm. Wildlife community: minnows, frogs, crayfish, etc. still present downstream from leak site and siphon dam. No visible evidence of wildlife kill. Barnett & Sons Excavating on site to dig up and replace flow line, and to finish removal of all contaminated soil. Excavating contractor instructed to dispose of contaminated soil at an approved landfill. Will follow up next day.

1907360333 2011-08-17 Production Wells No Violations Received notification of flow line leak on 8/17/2011 at 11:12 from Tom Basick of Enervest Operating (330-575-9248). Arrived on site at 13:00 8/17/2011. Flow line leak next to stream. Notified Ohio EPA (800-282-9378) of spill at 14:01. Instructed by EPA to handle spill with Enervests clean up crew - no EPA agent would be coming out. Met with Allen Summers of Enervest. His crew had all wells feeding into common tank battery shut in, placed absorbent pads on ground at leak site and stream bank, placed absorbent boom and pads approximately 61 meters downstream from leak. No visible oil skim on water surface downstream from boom. Leak caused by rotted flow line, Allen Summers reported approximately 0.5 to 1 barrel of crude oil/brine leaked from flow line. My recommendations were to (1) apply Peatsorb to leak site and local area around leak site, (2) have excavating crew remove all contaminated soil and vegetation, and dispose of at an approved landfill, (3) construct siphon dam along downstream side of natural pool, approximately 61 meters downstream from leak, (4) use water truck to vacuum oil skim accumulated in pool, (5) replace flow line from well head to tank battery. Will return in the morning to follow up.
