On the afternoon of 6.28.2018 I received a phone call from contract lease operator Jeremy Kohler. Mr. Kohler stated that he had adjusted the well head and stopped the leak. He continued that he had dug up the area under the puddle of production fluid and found a loose union on the dump line leading to the production tank. He stated he had tightened the fitting and cleaned the area of contaminated material. At the time of my follow-up inspection the well head leak was stopped. The area around the leak in the tank battery was cleaned up and left open. I did not observe any further leaking from the fitting. No further action is necessary. |

1470724815 | 2018-06-28 | Production Wells | Violations Noted | The purpose of my inspection was to conduct a status check on well #2-1288. RBDMS indicated the owner was current with reported production. Access to the well remained stable and drivable. The well was equipped with an electric pump jack. Production valves were open. At the time of inspection I observed gas leaking from the tubing head. Staining consistent with a legacy operation was present. Correct identification was present. A stable earthen dike was present around the equipment. The tank battery was located adjacent to a tributary to Berlin Reservoir. Equipment on location consisted of one (1) horizontal separator, one (1) production tanks and one (1) concrete brine vault. The separator remained in good repair and contained a current 7 day production chart. The tank remained in good repair. Valves were closed and plugged. The brine vault remained in good repair above grade and was holding fluid with freeboard. While on location I witnessed a puddle of crude oil approximately 18 in diameter located near the dump line between the separator and production tank . The fluid was fresh but not mobile. I also observed intermittent bubbles traveling up through the oil. While on location I called the listed emergency number and spoke with Mark Lyberger. I communicated the well head leak and dump line leak to Mr. Lyberger and notified him that I was issuing a compliance notice for the tank battery leak and requested immediate attention. Mr. Lyberger reported that he would have contract operator Jeremy Kohler make necessary repairs that day. I will schedule a follow-up inspection to document the status of repairs. |
