0581081946 2015-10-27 Exempt Well Pre Transfer No Violations I was on site to conduct an inspection of this well with DOGRM inspector - John Fleming. The wellhead consisted of a 4.5 inch casing with inch and a half tubing plumbed into a domestic line with a regulator and drip bottle. There was a mechanical separator adjacent to the wellhead and the tank battery consisted of a 100 bbl tank and 1500 gallon poly brine tank. The tank battery was diked. The well is being used for domestic use only and is approximately 305 feet from a dwelling. The wellhead, tank battery and access road locations were stable. The well is properly identified with a clear, legibly marked well identification sign mounted on the storage tank. The landowner has been contacted and has agreed to keep all production equipment on the site. The domestic transfer may proceed.
0957908940 2015-03-11 Production Wells No Violations I was on location for a status check. During my inspection, I found the well in production. The well is equipped with a plunger lift assembly. The flow line has two (2) ball valves in the open position. The tank battery consists of a 100 barrel production tank, a 50 barrel, poly brine tank and a mechanical separator. There is a dike around the two (2) tanks. There is an identification sign posted on the tank with John Diver listed as the well owner. There is a 1 inch domestic gas line coming off of the 2 inch flow line at the separator. The domestic line had a ball valve in the open position. The lease road is in stable condition. Upon further research of our RBDMS database, I found Chiefs Order 96-242 listing John and Norma Diver as the owners of the well. D & L filed zero production from 2011 through 2013. Photos were taken and are available upon request.
1150168044 2010-05-27 Production Wells No Violations I performed a random field inspection. I GPSd both the tank battery and the wellhead for the first time. The ID sign was not posted. The Pressure gage on the production line was 600 psi. The visual inspection of the rabbit well showed signs of a domestic well only. I called Dave Rice from D & L Energy(330-565-0430) and let Dave know about the no ID sign. The RBDMS showed the last production recorded was 1988. D & L Energy bought the well 1/13/2003 from United Petroleum Corp. He was in the area and would meet with me. Dave had talked with the landowner about the well and the landowner stated that the well was theirs since they can remember. I told Dave to get the Form 7 for a transfer for domestic use. D & L Energy and the landowner need to decide which one owns this well. I will monitor the results. There were no violations at this time.
1507821970 2018-03-29 Production Wells No Violations The purpose of my inspection was to conduct status check on well #2-0762. The well was utilized for domestic supply. Access to the well and tank battery remained stable and drivable. The 8.625 was below grade. The 4.5 production casing was equipped with 1.5 tubing topped with a master valve. The well was produced through a horizontal separator. The separator remained in good repair. No leaks were observed. No production chart was present. A stable earthen dike that was free of fluid was present around the production tanks. Tanks inside of the dike consisted of one (1) 100 bbl steel production tank and one (1) 1200 gallon poly brine tank. The steel tank remained in good repair and contained correct identification. Valves were closed and plugged. The poly tank remained in good repair. No violations were observed during this inspection.
