0062898656 2018-03-08 Production Wells No Violations The purpose of my inspection was to document the status of well #20631. ODNR-DOGRM Northeast District Supervisor Stephen Ochs was present during the inspection. Access to the well head was stable and drivable. Correct identification was posted at the well head. The 8.625 well head was at grade. The head ring was loose. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. No leaks were observed. A domestic supply connection was present. The tank battery was shared with #20636. Equipment on location consisted of one (1) vertical separator with a production meter and one (1) 140 bbl steel production tank. The separator remained in good repair. The meter house contained a current 7 day chart. A stable earthen dike that contained frozen water was present around the production tank. Correct identification was posted on the tank body. The tank remained in good repair. Valves were closed and plugged. No violations were observed during this inspection.
1187417597 2009-02-23 Production Wells No Violations I performed a random field inspection. I GPSd both the tank battery and the wellhead for the first time. I continued to walk around both the wellhead and the tank battery and found both to be in good shape. The ID signs were posted. There were no violations at this time.
