0193764616 2018-08-14 Production Wells No Violations The purpose of my inspection was to follow up on repairs to the tank battery of well #2-0622 in response to complaint #5919. I spoke with Enervest field superintendent Larry Lower on 8/13/2018. Mr. Lower reported the production tank was replaced, along with removal of a spare brine tank and improvement of the dike. At the time of my inspection I found a replacement tank with correct identification. The valves were closed and plugged. The hatch lid was closed. A 50 bbl poly brine tank was plumbed to the production tank. The inlet valve was closed. The dike around the equipment was improved. I saw no evidence of pollution or contamination on the ground inside of the dike. A pair of dead trees on the West side of the dike were broken off to prevent an overhead strike on persons or equipment within the battery. Following the inspection I spoke with the complainant regarding the work. Mr. Amey was satisfied with the work and agreed the complaint could be closed.
0841119173 2009-02-24 Production Wells No Violations I performed a random field inspection. I GPSd both the tank battery and the wellhead for the first time. I continued to walk around both the wellhead and the tank battery and found both to be in good shape. The ID signs were posted. I found no violations at this time.
1393072500 2018-08-02 Production Wells No Violations The purpose of my inspection was to respond to complaint #5919. Landowner Jamey Amey reported the well and equipment on his property were leaking an oily substance into the ground. Lease operator Michelle Suciu was present at the time of inspection. Ms. Suciu stated that Enervest management was aware of the failing production tank. She continued that the well head valves had been adjusted to prevent fluid production. The tank had been emptied and scheduled for replacement once surface conditions were dry enough to prevent extensive disturbance. Access to the well remained drivable and stable through a residential yard. The 8.625 well head was above grade and closed to atmosphere. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. Correct identification was present. No leaks or evidence of leaks were witnessed at the well head. The earthen dike around the production was worn and free of live or mobile fluid. I observed both dead and live vegetation bordering the dike. The condition of the vegetation did not indicate a defined area or path of pollution or contamination. Equipment on location consisted of one (1) horizontal separator, one (1) 100 bbl production tank, one (1) 50 bbl brine tank and one (1) steel brine tank. The separator remained in good repair and contained a current 16 day chart. I witnessed where the production tank had a small area at the base on the West side that had been leaking fluid. Absorbent diapers were laid down to absorb any residual product. At the time of inspection I did not witness any live or fresh fluid. Valves were closed and plugged. The poly brine tank remained in good repair. The steel brine tank was not in use. Following the inspection I spoke with Enervest field superintendent Larry Lower. Mr. Lower stated that the tank was scheduled to be replaced as soon as a crew completed work on another location. He continued that the dike would be impr
