Thompsons Well Service replaced the top joint of 2-3/8 tubing, as well as replacing the Demco valve, the rabbit catcher, and the ball valves. Thompsons then swabbed the excess fluid from the well. The gas leak is stopped and the well is now within compliance. The repairs satisfy ORC 1509.20 and OAC 1501.9-9-05(A)1. |

RBDMS indicated reported production was current. Access remained drivable and stable. The 8.625 well head was above grade and open to atmosphere. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. No leaks were observed. The tank battery consisted of one (1) horizontal separator, one (1) 140 bbl production tank and one (1) 1100 gallon poly brine tank. The separator remained in good repair and contained a current 7 day chart. A stable earthen dike that was free of fluid was present around the tank. The tank remained in good repair and contained correct identification. The sign was peeling from the tank but remained legible. Valves were closed and plugged. The poly brine tank remained in good repair. Valves were closed and plugged. No violations were observed during this inspection. |

At 09:00 on the day of the inspection I contacted Rick Huffman (330-904-2576) of Eric Petroleum, who agreed to dispatch pumper immediately to blow well down again. I conducted a field inspection of the well at 13:15, and found that the well was still leaking gas from around a metal clamp patch on the 2-3/8 tubing, just above the 4-1/2 well head. This issue has been ongoing since 01/11/2012. |

There was a leak in the 2.375 tubing just above the 4.5 well head. I witnessed the well being completely blown down into the stock tank. There was zero pressure on the well. Eric Petroleum is to have a service rig on site by 01/20/2012 to replace the top joint of 4.5 casing and 2.375 tubing as needed. Proper identifcation is posted. The tank battery dike in good condition, free of debris, with no spills or leaks around stock tank. There is no evidence of impact around well head. Eric Petroleum will notify me upon completion of the well service for a follow up inspection. |
