0203188746 2018-06-20 Orphan No Violations The purpose of my inspection was to document the status of well #2-0039. At the time of my inspection, I was accompanied by landowner Mary Altiere and her sons David and Chris. Access to the well was by foot through woodland. No identification was present. The well consists of 8 3/4 o.d. casing (surface) and 5 1/4 casing (production) on a hanger. The production casing was rusted through below the hanger and open to atmosphere. I did not witness any fluid or gas leaking from the casing. A flange cap is attached to the top of the production casing and it contains tubing that was likely utilized to monitor pressure. 2 tubing is plumbed out from the production casing and into a casing drip lying on the ground. No other equipment was present. No one could remember witnessing the well being operated. I will request ODNR-DOGRM surveyors verify the location against the existing plat. I will complete an idle well report for well #2-0039.
