1210419038 2008-05-08 Orphan No Violations Met with Ron Gibson and Hertsel Mayfield at well site and uncovered an old shale well 8 west of Our Lady Queen of Peace Church foundation. Columbia Gas of Ohio had readings of gas inside church wall and in parking lot. Gas service was shut off at church until venting of well. The well has a 3- bolt cap and valve leading to a home owned by Hertsel Mayfield at 696 Erie St. in Grafton. Ron Gibson blew pressure down and installed a temporary vent on well. Gas readings have diminished at church foundation. Columbia Gas was notified of venting. Casing on well is pitted and could begin leaking under vent. I recommend well be plugged under emergency I&O funding.
