Well is Ohio shale well.

1062779411 2003-09-05 Production Wells Violations Noted Requested by Grace Penzvalto to inspect a domestic gas well at 4620 Jaycox Rd. in Avon. The property is for sale. The well is located in rear of home in a wooden 3x 3 box. It is equipped with 1.5 tubing above ground with valves leading to home. No leakage of gas around well or lines. Pressure gauge shows 60 psi. at well top. Well is not registered with Department of Natural Resources, Mineral Resources Management. I advised Mr. Christ to have well registered with Division. I will send documents to register well. New home owners will have to transfer ownership. Well will need a permit number assigned by Division of Mineral resources.

Columbia gas was called to check for a gas leak at this residence and discovered an old domestic gas well in front yard of residence. Two areas of dead vegetation in yard between well and home. Columbia shut the valve on domestic well and gas has dissipated. 0% lels in yard. Domestic well is not bonded with the DMRM. Sent a letter to Mr. Stubenhoffer advising him to repair leak and bond well or have well plugged at his expense. From my inspection gas is leaking thru line suppling gas from well to home. Mike the township is Avon not Black River.

1121352425 2005-07-14 Field Inspected, Well Not Found No Violations Called by Capt. Saulter of Lorain Fire Dept. to investigate a domestic gas well location at 1178 North Ridge Rd. in Lorain. I met with Tina Sun to try and locate well. Fire Dept. said well was located inside garage. I found a water well cistern in garage under a metal lid. No gas leakage was detected on premises at this time. 1 gas line in basement has been shut in. Sent orphan well contractors list to Tina Sun. I will assist contractor in locating well on property. Fire dept. report is attached. Hot ,90 degrees. Fire Dept. # 440-204-2219.
1124450182 2005-08-18 Non Well No Violations Follow up to complaint # 05-008. After further investigation it has been determined that the 12 gas line was abandoned by the Ohio Fuel Gas company. Mr. Herman discovered the line on his property while draining a pond. Before notifying anyone, he drilled a hole in the line thinking it would drain the pond. Watert entered the line and created air pressure that return thru the hole he drilled. It was not gas pressure. The line is now dead. Contacted Columbia Gas and they agreed on the conclusion.Case closed. Hot, 88 degrees.
1125495703 2005-08-31 Non Well No Violations Met with Bill Barrett, owner of Barrett Construction. On 8-4-2005, Alliance Drilling was drilling a 42 diameter caisson for Cingular cell phone tower. At 33 6 they encountered a pocket of natural gas at 100% lel. After 24 hours the gas remained at 100%lel. Alliance Drilling filled the hole and tamped to 5 from surface. Bill Barrett notified ODNR, Uniontown office. Harold Moravy, Mineral Resource Inspector, responded on 8-31-2005. GPSd boring location and found 0% lel in original hole. No bubbling in the bore hole after rains from Hurricane Katrina. Researched 3-D Topo Map and found no old wells were drilled in the vicinity. Area is wooded and north of railroad. Brownhelm Township in Lorain county has pockets of natural gas trapped in the Ohio Shale close to surface. Recommend drilling above the shale in this area to alleviate the gas problem.
1126783318 2005-09-14 Non Well No Violations Called by Dave Hodges to investigate an odor complaint at 6340 Mallard Creek Run. Reviewed 3-D Topo map and found no oil and gas wells in 1 mile radius of complaint. Arrived at residence and met with Mr. And Mrs. Zadorozny. They advised me of a strong odor in and outside of their residence occurs normally in the evenings. I smelled a musty or mold odor in their car and outside the residence near the foundation of the home. The house is built on a concrete slap in a wooded area. I found no evidence of an abandoned well near the foundation. The house was completed in July of 2004.

1149859213 2006-06-09 Non Well No Violations Case# 06-002. Lite readings of methane were recorded in back yard at 543 Augusta Drive in Elyria at the Dean DeMarco residence. Brought in a mini excavator and dug around area where methane was detected. Both areas are cut off Red Oak tree stumps that are decaying and producing methane. No well were discovered in back yard or on well maps. Mr. DeMarco is having stumps ground.

Bailing water from well at this time. Fluid level was at 100 when started bailing. Due to the construction I have approved the well to be plugged before permit is issued.Well is located 30 south of Lake Erie. Cold, windy, 10 degrees.
1173374006 2005-04-11 Non Well No Violations Requested by Oberlin Fire Chief, Dennis Karin, to invetigate gas readings in residences at 44148 and 44152. Readings at water well vent is 9% lel. Water well is located inside residence at 44148 and supplies both residences with water. Readings at the spigots are 3 to 4 % lel. Advised Doris Carney to have water well vented outside of the home. Fire Department has evacuated the residence at 44148. Doris Carney stated water has fissed for years. No oil or gas wells within 1/2 mile radius. Sending questionaires to Doris Carney.
1194545956 2007-11-08 Field Inspected, Well Not Found No Violations Called by Avon Fire Department Chief Frank Root Jr. Gas odor in sump at 3817 Swinton Circle in Avon. Met with Avon Fire Department personnel,Oster Homes, Michael and Tangja Finch ,Ron Gibson to look for natural gas well in area. Unable to find a well on the property. Tested vents and sump in home and LEL readings were 0 %. On 11-6-2007 homeownres smelled gas in basement of home. Columbia Gas of Ohio responded and advised Finchs to call fire department. Fire department stated they had a gas scope reading of 5 ppm. They notified CEI electric and had it disconnected and Columbia shut off gas. Finchs were put up in a motel by the fire department. On 11-7-2007 Norburt Lowder took readings of the sump and had 0% LEL. Avon Fire allowed finchs to return home. Cloudy, 35 degrees.
1243521558 2009-05-28 Orphan No Violations Met with Lewis Sigueroa at 203 15th St in Lorain City. He would not let me in to inspect well. Tenents have moved out and electric has been disconnected. I notified the City Fire Department. Mr. Sigeroa advised he will notify me to inspect well when he dicusses the well with a private contractor. I gave him a Orphan well pamphlet and contractors list. 0% lels at open basement windows. The well is supposably in the basement of the building.
1269984608 2010-03-30 Field Inspected, Well Not Found No Violations Called by Eaton Township Fire department and Spencer Kershner to help determine a pocket of natural gas at intersection of SR 82 and Giles Rd. in Eaton township, Lorain Co. Gas was encountered during digging of a new 36 water line at 14 underground. No visible wells or old wells in area. Gas concentrations are 100% lel at source and 40 % lel 3 above source and 0% lel at ground surface. 0 ppm H2S. Gas was diminishing as we tested. Fenced off area and will test on 3-31 2010. If gas is present will install a vent pipe so construction can be completed. Columbia Gas of Ohio advised it is not their gas or no service along SR 82. Sun, 56 degrees.

1270814360 2010-04-08 Non Well No Violations Case # 10-001, Inspected gas vent at Giles Rd. and SR 82. Sewer project is being back filled. Gas in vent has dissipated to 0% lel. Project engineer advised no readings for past two days at vent. Removed vent pipe and back filled area.
1278160463 2010-07-03 Orphan No Violations Called out by Assistant Fire Chief Gary Burrows, to investigate gas leak inside building at 2831 Globe Ave. in Lorain City. Readings inside one area of basement in building is .15% CH4 and 3 ppm H2S.Building has been evacuated. Found gas readings in back yard at 1705 E 29th St. Also found a domestic gas well in back yard at 1709 E 29th St. It is leaking gas around casing in well pit. Both properties are owned by Hulio and Alice Rivera. Called Siteworks to dig up area at 1705 E 29th St. Fire Department and Columbia Gas has left scene. I will keep fire department notified. Located gas leaking at 30 % lel on rear of property at 1709 E 29th St. called Columbia Gas to expose service box. Box was leaking 48% lel. Columbia Gas replaced the valve. We will monitor gas in building at 1709 E. 29th and 2831 Globe Ave on Tuesday, 7-6-2010.

1709414227 2006-11-13 Non Well No Violations Requested by John Schafrath of ODOT to research area along SR 18, 2 miles west of Pitts Rd. His survey indicated an old pipeline in this area. Six old gas wells were drilled between 1936 and 1952. The wells have been plugged but the old gathering line remains. ODOT is planning on working in this area and needed to know if line is active.

Well is equipped with 5 casing and regulators leading in building. Odor of sulfur near well but no reading on gas scope, 0% lel. Approximate depth is 700 to 800 in Ohio shale. Mr. Danus advised a private contractor will plug the well in the future. Gave him a list of contractors.
