1041304201 2008-07-02 Production Wells Violations Noted Called in by Dayle Payne of Columbia Gas. During her line inspection she found a leak from well to home at 35615 Chestnut Ridge Rd in North Ridgeville township. I met with Craig Praszek ( 440-213-9480 Bus.)and located the domestic gas well his dad uses to heat his home. They hired a contractor to repair leak. I advised him the well needs bonded and insured. Marlene has mailed a domestic well owner packet to Mr. Praszek. I advised them to post identification at well site. Advised Dayle Payne of findings. Sun, 80 degrees.
1218393279 2008-08-10 Production Wells Violations Noted Called by Columbia Gas of Ohio hotline to inspect a odor of gas leaking from a domestic gas well owned by Albert Praszek. I responded to site ,with N Ridgeville Fire Department. Odor only, away from well head. Issued an NOV on 7-2-2008 for failure to bond and no identification. On 8-7-08 Dayle Payne of Columbia Gas was notified of odor of gas leaking from well casing. I notified Craig Praszek to have well repaired an in compliance. Siteworks Drilling is repairing leak today 8-10-2008. Odor of gas is present, but no lels away from well head. Well had 25 psi on it before blowing it down to repair. Neighbor to east is calling in odor complaint to Columbia Gas. His home is 50 from gas well. Wind is blowing towards his home out of the west.
1218731549 2008-08-14 Production Wells No Violations Gas leak has been repaired and change of owner papers in process. Advised Craig Praszek to post identification at well head. Advised him to back fill hole around well head.
1339089237 2012-06-07 Production Wells Violations Noted I inspected the site at the request of Mr. Praszek, he is to sell property and wants an inspection report for the well.

1. Well Identification is not ledgable and needs to be reposted. 2. The new landowner needs to call Erica Freeman (614-265-6900) to file updated bonding forms, post updated insurance and filing fees.

I called Howard/ Hanna at 440-258-8717 and spoke to the listing agent. I advised her that the new owners will have to meet current rules and laws for ownership of a domestic well and that she should call Erica Freeman, at 614-265-6900, to get forms for filing for ownership.
